Research On The Effects Of Tracheotomy At Different Time In Patients With Brainstem Injury

Objective:To study the therapeutic effect and the influence of the treatment process for brain stem injuried patients with descending tracheotomy at different time. Methods: choose79cases of brain stem injuried and also been tracheotomied, divided into A, B two groups, A group of37patients with the approval of the patients’ families,been tracheotomied within72hours of brain stem injury; B group of42patients, with the approval of the patients’ families,been tracheotomied after72hours of brain stem injury;other treatments of the two groups were the same.Compared two groups’ dates, to study the therapeutic effect and the influence of the treatment process of patients with brain stem injury been tracheotomied at different time. Result:Of the therapeutic effect, the differences between A and B group-patients have no statistcal significance in awake rates, continuous coma rates and mortality rates(P=0.657). Compared to the A and B groups of patients on different etiology, the brain stem injury caused by cerebral hemorrhage patients in the treatment effect between different reintubated timing difference was statistically significant (P=0.011), brain stem injury caused by traumatic brain injury patients in the treatment effect between different reintubated timing difference was statistically significant (P=0.001). From the treatment process, in the final awake patients, no statistical significance of different time for patients with tracheotomy awake. In patients with sustained disturbance of consciousness, different timing of tracheotomy patients’ Glasgow score recovery had no statistical significant (P=0.153). In patients eventually died, the different timing of tracheotomy patients with lower survival time difference compared to P value (P=0.057) close to0.05, due to the small sample size, sample size needs to be further expanded, before any conclusions. Conclusion:Not all patients with brainstem injury can benefit from the early tracheotomy. The therapeutic effect of tracheotomy at different time for patients of brainstem caused by different causes may exist differences. The couse of time for awake in those awaked patients have no differences in tracheotomy time chose. Tracheotomy for different continuous disturbance of consciousness of patients, Glasgow grading recovery is probably no difference.
