| Objective:To explore the correlation between the ultrasonic performances of papillary thyroid carcinoma (PTC) and cervical lymphatic metastasis. Methods: Ultrasonic performances of152patients with158PTC were analyzed before surgery, and divided into metastasis group (n=55cases,61PTC) and without metastasis group (n=97cases,97PTC) according to the pathology whether there was cervical lymph metastasis. Single factor analysis was used to evaluate the correlation between the PTC ultrasonic performances and the cervical lymph metastasis. And a Logistic regression model was established by cervical lymphatic metastasis diagnosed by pathology as dependent variable, the ultrasonic performances of PTC as independent variable to multivariate analysis. Results:Single factor analysis shows that the cancer nodule size、aspect ratio、 the border、the contact area between the border and the thyroid capsule、calcification and blood type classification and cervical lymph node metastasis has some relevance, the difference was statistically significant (P<0.05); Then Logistic regression shows that the cancer nodule size-. the contact area between the border and the thyroid capsule calcification and blood type classification are risk factors for cervical lymph node metastasis, the difference was statistically significant (P<0.05). The more meaningful four ultrasonic performances comprehensive analysis found that when the tumor ultrasound examination in line with the performance of one of≥3item performance when predicting the occurrence of PTC sensitivity cervical lymph node metastasis was83.6%, specificity was88.7%. Conclusion:PTC ultrasonic performances, the cancer nodule size^the contact area between the border and the thyroid capsule、calcification and blood type classification cervical lymph node metastasis, and comprehensive analysis, PTC nodules ultrasonic performances can better predict cervical lymph node metastasis, preoperative prediction whether PTC cervical lymph node metastasis of great value. |
