Objecitve The contents of the ingredients in Naru-3pill was determined to research ifthe quality was stable or not. By studying the effect that the Terminalia and alcoholextract of Piper on liver medicine enzyme CYP3A4and CYP2D6to explore theattenuation of two medicines on Kusnezoffii toxic ingredients. The efficacy of Naru-3pill on AA rats was observed, providing experimental evidence for its safe using.Methods1High performance liquid chromatography (HPLC) method was used todetermine the main active ingredients in Naru-3pill.2SD rats were randomly dividedinto control group, Terminalia induction group, Piper longum induction group. Thecontrol group was given normal saline every day. Terminalia induction group was givenalcohol extract of Terminalia (250mg/kg). Piper longum induction group was givenalcohol extract of Piper longum (130mg/kg) respectively). Induction of7days in arow,then to give the mixture of metoprolol(25mg/kg) and dapsone (20mg/kg) at theeighth day.And drawing blood at0.13,0.25,0.5,1,1.5,2,2.5,3,5,8,12,24h, to determine the metoprolol and dapsone drug concentration of plasma in each groupby the established HPLC.37SD rats were randomly selected as control group controlgroup, the rest for establishing AA model. After the successful model rats were randomlydivided into model group, high dose group and low dose group. high-dose group and low-dose group and then administer the drugs to rats for2weeks(the rats in control group andmodel group were administered saline and the others administered the drugs of differentdosages repsectively),determining the content of IL-1at15thday.At last,to kill the ratsand color the ankle joints in order to observe the histopathology changes and injuries ofcartilaginous tissue.Results1The linear range of piperine was12~60μg·ml-1, which accounted for0.5490%; Then that of galic acid was0.075~0.375μg·ml-1, which occupied0.0724%;And that of aconitine was0.021~0.750mg·ml-1, which accounted for0.0033%.2Tocompare with the control group, the AUC as well as Cmaxof metoprolol increased26.39%(P<0.05) and14.92%(P<0.05) respectively while the level of CL decreased20.87%(P<0.05) after the rats were administered Terminalia chebula Retz. Moreover, the Cmaxofaminophenylsulfone increased17.13%(P<0.05) after the rats were administeredTerminalia chebula Retz. Then it showed that after administered the rats Chaviacaroxburghii, the AUC and the Cmaxof metoprolol decreased25.54%(P<0.05) as well as 23.06%(P<0.05) respectively while the level of CL was reduced by34.43%(P<0.05).Furthermore,the AUC of Chaviaca roxburghii decreased32.76%(P<0.05) while the level CL increased48.08%(P<0.05).3The level of anti-CⅡofinflammatory rats determined at the15thand30thday respectively in the control groupwas less than that in model group (P<0.05) and the successful rate of making models was84%.4After curing the rats, it showed that the index of AI on rats in the high-dose groupwas lower than that of model group at the10thday(P<0.05) and at the14thday, it turnedout that the result of the high-dose group was the same as the one of the10thdaycompared with the model group(P<0.05) while compared with the low-dose group theindex of AI declined obviously.Then the level of IL-1in rats of high-dose group and low-dose group was higher than that of control group(P<0.05) while less than that of modelgroup(P<0.05).5The scores of synovial tissues and cartilage in high-dose group werelower than that in low-dose group and model group(P<0.05).Then according to thepathological changes of cartilage,it was observed that the surface of was thick andswelling as well as the cells was in disorder and little.In addition,it induced the fibrogenicresponse and many inflammatory cells.Howerever,the test groups improve thepathological changes especially high-dose group.Conclusions1In selected chromatographic conditions, the the measured quality ofNaru-3pills was stable.2The HPLC method established for determining the contentsof metoprolol and dapsone was accurate and reliable, which could be used to determinethe the contents of metoprolol and dapsone in vivo and in vitro. The results showed thatTerminalia could inhibit the liver medicine enzymes CYP2D6and CYP3A4, while Piperlongum could induce the liver drug enzymes CYP2D6and CYP3A4. Thererfore,itsuggested that Terminalia didn’t did not play attenuation to the toxic components ofKusnezoffii. Piper longum accelerated the metabolism of toxic ingredients of Kusnezoffii,play attenuation.3Naru-3pill could relieve the inflammation, eliminate swelling andimprove the injury of pathological changes. Also, it could reduce the content of IL-1ofthe serum and protect the cartilaginous tissues, proving Naru-3pill has a certaintherapeutic effect to the AA rats.
