Objective Investigate the correlation analysis of MSCT in liver volumemeasurement and Child-Pugh classification of hepatitis B cirrhosis.Methods Totally one hundred and one cirrhosis of hepatitis B cases were selected,all subjects were accordingly liver function grades divided into three groups,Child-Pugh A grade31cases, Child-Pugh B grade42cases, Child-Pugh C grade28cases, average age is52.72years. Selected Standards:①All cirrhosis ofhepatitis B patients were diagnosed by ultrasound B,CT or other radiologyimaging methods.②All patients had not complicated liver benign or malignanttumor.③All patients had not complicated portal vein embolism and portal veincavernous transformation.④All patients had not complicated other hepaticdiseases.At the same time, ninety eight adults having neither liver disease (44male and54female) nor other diseases leading to liver volume change are selected, whichwere control group, all subjects were divided into four groups, Group A(age range18to40), B (age range41to50), C (age range51to60), D (age range61to83). Selected Standard:①All cases had not hepatic diseases.②All patientshad not complicated liver benign or malignant tumor.③All cases had notcomplicated other system diseases, for example bile duct diseases, pancreasdiseases, HCC or metastatic tumor, infections of liver, cardiac diseases, kidneydiseases, blood diseases, lymphatic system diseases, connective tissue diseases,metabolic disorders, autoimmune disorders, parasitic diseases, and unexplained fever for a long time.All subjects underwent Siemens Somatom Definition dual-source Multi-spiralCT enhancement examination. The margins of liver lobe was outlined manuallyusing volume analysis software of Syngo Acquisition workstation, and the volumeof the liver lobe was calculated automatically by software.Results There were no significant difference between male and female in thevolume of each lobe of liver and the whole liver volume (P>0.05). There weresignificant differences in male’s caudate lobe, whole liver volume, female’smedial segment of liver left lobe, right anterior lobe of liver and whole livervolume (P<0.05).There was negative correlation between the whole liver volume and age(r=-0.408, P<0.01).However, there were no significant correlation between livervolume and gender. Whole liver volume was positive correlation with body weightand BSA (BSA:r=0.254,P<0.05;BW:r=0.421,P<0.01), however,there was nocorrelation between liver volume and body high (r=0.15,P>0.05).Compared the cirrhosis group and the control group, left lateral lobe of cirrhosisliver is bigger than left lateral lobe of normal liver,the difference of the two groupis significant(P<0.05), cirrhosis liver caudate lobe is also bigger than normal liver,the difference of the two group is significant (P<0.05). However, the medialsegment of liver left lobe, right anterior lobe and right posterior lobe of cirrhosisliver were smaller than normal liver, and the difference of the medial segment ofliver left lobe and right posterior lobe were significant(P<0.05). And the totalliver volume of cirrhosis liver is also smaller than normal liver, the difference ofthe two group were significant (P<0.05).The three groups of cirrhosis were detected, on the whole, the liver volumereduced with the cirrhosis order of severity. The left lateral lobe, right posterior lobe and total liver volume is significant difference between Child-Pugh A and B,C groups (P<0.05), the difference of medial segment of liver left lobe issignificant in each group (P<0.05), and the difference of right anterior lobebetween Child-Pugh A and C is significant, and the difference of caudate lobebetween Child-Pugh A and B is significant(P<0.05).There is positive correlation between total liver volume and Child-Pughscore(r=-0.223, P=0.025).Conclusion The normal whole liver volume changes can be effected by with age,BW,BSA.To compared with normal liver, the cirrhosis of hepatitis B liver’s left laterallobe and caudate lobe is hyperplasia, medial segment of liver left lobe, rightanterior lobe, right posterior lobe and total liver volume is atrophy.Cirrhosis liver volume is reduced with the disease severity.In short, liver volume measurement can assess liver function, and will havepositive impact clinical treatment and surgical operation.
