ObjectiveThis study is to learn the integrity of the cranial nerve fiber wih obsessive-compulsive disorder(OCD) patients by using DTI(diffusion tensor imaging) of MRI.To explore the correlation between diffusion tensor of the abnormal brain regions,course of OCD patients and clinical symptoms.To provide imaging data for OCD patients at etiology research and clinical diagnosis.MethodAccording to the diagnostic criteria of OCD in Design Standards Manual(DSM-IV),collected 34 cases incipient OCD patients and 31 healthy volunteers as healthy control group.All the research object completed psychiatric scales and DTI scans at the first day.Using double sample t test to analyze the difference of fractional anisotropy(FA) between the OCD group and healthy control group.Using Pearson correlation to explore the correlation between FA of cerebral white matter fibers in patients,clinical symptoms,course of diseases,age and the number of years for education.The threshold was set 0.05 in all the statistical test(P<0.05).Result1.The comparison results of OCD group and healthy control group on the general information There were no statistical differences between two groups in age(t=1.132,P=0.175),gender(χ2=0.844, P=0.124) and the number of years for education(t=-1.679,P=0.901).2.The diffuse anomaly area of cerebral white matter fibers of OCD patiensThe comparison results of FA value of corpus callosum between OCD group and healthy control group shew that the fractional anisotropy of splenium of corpus callosum of OCD group was higher than healthy control group(t=1.697,P=0.012). The comparison of FA value of the cerebellum nerve fibers between OCD group and healthy control group shew that the FA value of the right side of the cranial nerve fiber of OCD group was lower than healthy control group(t=2.234,P=0.031).3.FA value of splenium and knee of corpus callosum of OCD group had positive correlation with Yale-brown obsessive-compulsive scale(Y-BOCS) score(r=0.421,P=0.015;r=0.425,P=0.013).FA value of left and right cerebellum of OCD group had negative correlation with Y-BOCS score(r=-0.392,P=0.028;r=-0.485,P=0.043). 4.FA value of splenium of corpus callosum of OCD group had positive correlation with obsessive thought(r=0.352,P=0.03).While FA value of right cerebellum had negative correlation with compulsive behavior(r=-0.430,P=0.01).Conclusion1.The incipient OCD patients shew enhanced diffusion state in nerve fiber of splenium corpus callosum in DWI of MRI,while shew lower diffusion state in right side of cerebellum.2.For incipient OCD patients,the higher the FA value of splenium of corpus callosum was,the more obviously the obsessive thoughts was.At the same time,the lower the FA value of right side of cerebellum was,the more obviously the compulsive behavior was.
