Objective:To study the difference of brain activation under the language task between patients with temporal lobe epilepsy and the normal people by Blood oxygen level dependent-functional magnetic resonance imaging, to explore the language impairment of the temporal lobe epilepsy patients.Methods : Seventeen right-handed cases of TLE proved by operation and pathology and Fourteen right-handed healthy volunteers performed verbal generation language task, while the BOLD-f MRI examination underway. The patients and healthy control subjects were obtained language IQ and total IQ score by Wechsler adult intelligence scale-revised in china(WAIS-RC). The BOLD-f MRI brain image data were analyzed by SPM8, and two samples of t test were used to compare the difference of language features and the brain activation of the task graph between the epilepsy group and the control group.Results:1、Cognitive function results: the language IQ and total IQ Score in temporal lobe epilepsy patients were significantly lower than the mean 1evel of healthy control subjects(P<0.01).2、Language BOLD-f MRI activate map results: In verbal generation task, the left temporal epilepsy compared with healthy control subjects,the positive activated differences brain regions include right precuneus region, negative activated differences brain regions include bilateral inferior frontal gyrus, inferior temporal gyrus, bilateral occipital lobe, bilateral cerebellar hemispheres, bilateral anterior cingulate gyrus, insula, supplementary motor area and the left hippocampus, lingual gyrus(P < 0.01).the left temporal epilepsy compared with healthy control subjects,the negative activated differences brain regions include right inferior frontal gyrus, right postcentral gyrus and right insula. 3 、Laterality index(LI):The LI between paitents and the control subjects no significant difference, but the mean LI in patient with left temporal lobe epilepsy was apparently lower than the health controls, and obviously appear right hemisphere superiority and bilateral hemispheres dominance.Conclusion: The language and cognitive function was significantly reduced in patients with temporal lobe epilepsy compared with health controls. The BOLD-f MRI brain activation range and intensity of left temporal lobe epilepsy significantly lower than the healthy controls. Left temporal lobe epilepsy patients have atypical dominant hemisphere lateralization.
