Objective:Discuss The occurrence of fragmented QRS complex through the analysising of body surface electrocardiogram in patients with acute myocardial infarction,To study the predictive value of fragmented QRS complex(f QRS)for Acute myocardial infarction and the relationship between Fragmented QRS complex with the severity of coronary artery lesions.Methods:Giving detailed analysis of the electrocardiograms what admitted within 24 hours after in hospital about 963 cases acute myocardial infarction patients(AMI), and dividing into two groups according to the presence of f QRS wave. these patient’s age, sex, etiology, duration and coronary vascular lesions and so on were compared between the two groups. Cardiac events( Recurrence of angina pectoris Hospitalization for cardiovascular events again cardiac death allcause mortality) were recorded in all patients during 12 months follow-up. The degree of stenosis and the extent of lesions were analyzed and compared for patients who suffer Percutaneous Coronary Intervention treatment. the severity of coronary lesion of these patients was evaluate Using SYNTAX integration. Analyzed the telationship between f QRS with SYNTAX integration and Cardiac events by Logistic regression analysis. Using Kaplan Meier survival curve analysis the all-cause mortality.Results: There were no statistical difference Between the two groups about the basic conditions(gender, age, course of the disease, treatment, smoking, and if there is a history of high blood pressure, diabetes, hyperlipidemia). The occurrence of f QRS wave in the group of ST-elevation myocardial infarction are more than the group of No-ST-segment elevation Myocardial infarction. The probability of recurrence of angina pectoris, hospitalization for cardiovascular events again, cardiac death and all-cause mortality in FQRS group was significantly higher than non-f QRS group. When we performed multiple logistic regression analysis, FQRS was found to be related to Cardiac events. Kaplan Meier survival analysis revealed the survival rate was lower in fragmented QRS group than that in non-fragmented FQRS group.Conclusion: FQRS appear in body surface electrocardiogram of patients with acute myocardial infarction is a sign of increased cardiovascular events. It has a significant predictive value for the long-term prognosis of patients, f QRS can serve as an indicators to identify people at high risk of acute myocardial infarction.
