| Objective: Irritable bowel syndrome(IBS) is a functional bowel disease with the characteristics of abdominal pain or discomfort accompanied by alteration of bowel habit or defecation, and with features of disordered defecation. It lacks morphological changes and biochemical abnormalities that could explain the symptoms. It is often persistent or recurrent, lacks clinically significant morphological and biochemical abnormalities; etiology and pathogenesis is not fully determined. The disease has always been a research hot spot and focus in the functional gastrointestinal diseases because it’s complex dynamics of recurrent sex, symptoms and treatments of no specificity. Most scholars believe that the pathogenesis of IBS is the result of comprehensive shaping of many factors such as gastrointestinal dyskinesia, high sensitivity, intestinal infection of viscera and the dysbacteriosis, mental, and social environment factors, etc. There is no specific treatment in modern medicine for the disease, and the long-term oral western medicine adverse reaction is more and the patient compliance is poor. Irritable bowel syndrome is a difficult problem in the treatment at home and abroad for decades. A number of studies show that the disease is the advantage of TCM treatment of diseases; traditional Chinese medicine formula curative effect is stable, convenient to use, cheap, in addition the intolerance in patients with adverse reactions is extremely low; the treatment is not recurrent for IBS patients, which can effectively shorten the treatment course, improve curative effect, and thus eventually cure the disease.The purpose of this study is to sum up the experiences of treatment of IBS–D with Shengyang Yiwei Decoction,( including huangqi, banxia, renshen, gancao, duhuo, fangfeng, baishao, qianghuo, jupi, fuling, chaihu, zexie, baizhu, huanglian, shengjiang, dazao). IBS–D belongs to the category of “gastrointestinal stagnation diarrhea abdominal pain”. The location of disease is in large intestine, with the relevant of liver, spleen and kidney.Spleen deficiency is an important risk factor. In recent years, with the improvement of people’s living condition, the adjustment of the diet, work and life pressure increases, people eat too much rich food and fat, so more and more patients who belong to the type of weak spleen and stomach appear. This flexible and subtract treatments are available on the symptoms.This topic focuses on the treatment of stomach syndrome IBS–D who belongs to hot and humid pixu(spleen deficient) type with shengyang Yi wei Decoction of clinical efficacy, safety and quality of life, and it provides a new treatment idea and effective method of knotty for the treatment of diarrhea type of IBS which is a clinical common and frequently-occurring disease.Methods: In this study, 160 definite patients of IBS–D who belongs to the “hot and humid pixu spleen deficient” type were randomly divided into treatment group which contains 80 cases and control group which includes 80 cases according to the order of seeing a doctor in a ratio of 1:1. The treatment group according to the different symptoms was given the warm sheng Yang Yi wei Decoction, twice in the morning and evening, before eating. Observe once a week, 4 weeks as a course of treatment. The control group was given the trimebutine maleate tablets, one tablet each time, 3 times a day, combined with bacillus coagulans tablets, 3 times a day, every time 3 pieces. The course of treatment was 4 weeks. We observed the two groups before and after treatment about the main symptoms improvement such as diarrhea, abdominal pain, abdominal distention, shit, shit character, and evaluated the quality of life of patients by the internationally used SF–36 scale and included in the statistical analysis the comprehensive evaluation of the clinical efficacy and safety of treatment.Results:1 The TCM syndrome total score of two groups after treatment was significantly decreased: the treatment group can lower syndrome total score better than the control group(P<0.01); The curative effect of two groups of TCM symptoms show efficiency of 87.50% and 51.25% respectively, and the treatment group is better than that of control group(P<0.01).2 Treatment group in easing main symptoms such as abdominal pain, defecate urgent, languid, less food and stay is better than that in control group(P<0.05), in easing the secondary symptoms such as mouth dry and bitter, anus heat, endless defecate, the feeling of the treatment group was better than control group(P<0.01), while in terms of improvement of diarrhea, abdominal distension, defecate character, two groups of curative effect is similarly.3 In the treatment group in the SF–36 scale, body pain, general health, social function and mental health of integral were obviously improved(P<0. 05), and is better than that of control group(P<0. 05).4 Two groups of patients with no obvious adverse reaction occurred during treatment. The patients of treatment group at the beginning only appeared symptoms such as headache, drowsiness, dizziness, but can be tolerated, and not terminated treatment.Conclusion: The curative effect of syndrome IBS–D who belongs to hot and humid pixu spleen deficient type with Shengyang Yi wei Decoction was reliable, total effective rate was 92.50%, and superior to western medicine control group. It was no obvious side effects, and patients compliance was good. It is a safe and effective treatment which can effectively relieve patients with diarrhea, abdominal pain, abdominal distension, the symptom such as urgent defecation, languid, and can improve the patient’s quality of life. |
