Objective TO explore the phenomenon of postsystolic shortening in patients with essential hypertension with different pattern of left ventricular by two-dimensional speckle tracking imaging, and discuss that how postsystolic shortening affect the asynchronism of left ventricular in patients with essential hypertension.Methods Sixty patients with essential hypertension were divided into two subgroups based on left ventricular configuration including Left ventricle normal geometric (LVN group, n=22) and Left ventricular remodeling (LVR group, n=38).30 normal subjects were enrolled. The end-diastolic LV diameter (LVEDd)、end-constriction LV diameter (LVEDs)、end-diastolic thickness of ventricular septum (IVSd)、end-diastolic thickness of LV posterior wall (LVPWd) were measured. Respectively collect three cardiac cycles by two-dimensional echocardiography from apical four-chamber、 three chamber、 two-chamber views. The images will be analyzed by importing to QLAB 8.1. Detect myocardial movement of LV by 2D-STI, as a result, LV were divided into 16 segments and speed-time curves were acquired. Record the peak velocity of contraction (Vs) and time to peak (Ts). We define PSS as positive wave at isovolumic relaxation phase on speed-time curves, which happened at the time between aortic valve closing and mitral valve opening. Than record the number of PSS and peak velocity of PSS (Vpss)Results Comparison of Vs between LVN and control group, there was no significantly different (P>0.05) Vs only in intermediate segment and apical segment of interventricular septum of LVR were increased compared to the controls (P<0.05), the other segments showed no significantly different (P>0.05). Ts showed no significantly different in hypertension groups compared to the control (P>0.05). The positive rate and the peak velocity of PSS in hypertension are increased than the control group (P<0.05), which showed as LVN group>LVN group>controls.Conclusion PSS in patients with essential hypertension reminder that there are relatively myocardial ischemia in parts of the myocardium. With higher positive rate and faster Vpss,, The greater the chance of myocardial ischemia.
