Objective To evaluate the variation of QOL and Cognitive function of patients with NSCLC undergoing chemotherapy at terminal stage and analysis its related influencing factors.Methods 41 NSCLC patients and 30 control subjects were recruited. Cognitive states were investigated by Montreal Cognitive Assessment(Mo CA)、Hamilton Depression Scale(HAMD) and Hamilton Anxiety Scale(HAMA). The QOL(investigated by QLQLC-43),Haemoglobin and Albumin were evaluated in NSCLC patients.To analyze the variation of the scores and its correlative factors.Results In spite of dyspnea symptom has relieved after 4 cycles of chemotherapy, physical domain、emotional domain,cognitive function Roles feature and QOL scores declined meanwhile;symptoms such as fatigue, nausea, vomiting, constipation, insomnia grew worse,;society domain, pain, diarrhea and lung cancer specific module scores have no obvious change.On the Mo CA scores, cognitive impairment(Mo CA score < 26) rate of advanced lung cancer patients before chemotherapy is significantly higher than the control group(control group 6.67% vs treatment group26.83%,P < 0.05), Mainly in the visual space and executive function;Mo CA scores significantly decreased after 4 cycles of chemotherapy patients( Before chemotherapy 26.80 ± 2.19 vs after 4 cycles chemotherapy 25.51± 2.12, P < 0.01), mainly in the memory and attention domain. And the fall has a moderate correlation with hemoglobin decline(r = 0.534, P < 0.01). On the HAMA and HAMD scores, the lowest HAMD and HAMA scores were found in health control group,while highest HAMD and HAMA scores were found in NSCLC patients after 4 cycles of chemotherapy.Conclusion The cognitive impairment has existed before chemotherapy of advanced NSCLC patients,chemotherapy can to some extent improve the patients quality of lifewith the cognitive impairment degree aggravating, related to the factors such as age, hemoglobin levels.
