Researching The Implementation Of DRGs Of Major Diseases Of NNMs In Anhui Province

Objective:Since 2010,the pilot work of DRGs of major diseases of NCMS has lasted for more than three years. A comprehensive understanding of the overall progress of the implementation of the policy and its effects, studying policy implementation, and to analyze the problems in the implementation process, propose appropriate policy recommendations, can provide a reference for improving and perfecting this system.Method:Survey of hospital’s diseases of the DRGs of maj or diseases of NCMS where the author internship, including simplicity of congenital heart disease (> 14 years old), complexity of congenital heart disease (> 14 years old), pituitary adenomas and heart valve disease, then select three hospitals which implement the same diseases, a total of four hospitals to investigate. From the four hospitals’medical service and health insurance get the major diseases of the DRGs of NCMS implemented three years (July 1,2011-June 30,2014) and the corresponding disease to the implementation of the previous year (July 1,2010-June 30,2011) of all cases of staying days, medical expenses and NCMS fund, and so on. Research the implementation effect of the DRGs of major diseases of NCMS, and analyze issues which arising in the implementation process. For who using the major disease macro-reforms reimbursement way, patients satisfaction questionnaire to understand patients cognition and satisfaction on the performance of the policy. For four kinds of hospital implementation of questionnaire survey to the related department staff, understand their understanding of the policy level and satisfaction.Results:(1) Compared with the traditional pay by project, the implementation effect of the DRGs of major disease of NCMS is obvious. (2) Medical cost control needs to be further strengthened. (3) Patients have low level of awareness and participation of the DRGs of major disease of NCMS.(4) Patient satisfaction mixed reviews, but the general level of technology for treatment and evaluation of a high sense of responsibility. (5) The staffs awareness of the DRGs of major disease of NCMS needs to be strengthened. (6) The policy of the DRGs of major disease of NCMS has affected the enthusiasm of medical side.Conclusion:(1) The implementation effect of the DRGs of maj or disease of NCMS is obvious. (2) Agricultural management departments and medical institutions shall perform on major disease diagnostic scheme. (3) Establish a mechanism for adjusting quotas.(4) Strengthen publicity and education, to improve the participation of farmers and medical cognitive level.(5) Build benefit compensation mechanism, reasonable compensation medical insurance interests.(6) In future studies, to expand the sample size, for more diseases more comprehensive investigation.
