Abstract/Summary:
Objective:Ventricular tachycardia (VT) following repair of congenital heart disease is difficult for catheter ablation. Several reports had described VT following repair of tetralogy of Fallot. However, there is still a lack of data regarding ventricular tachycardia after surgical treatment of pulmonary stenosis with intact ventricular septum.Here we report a patient with VT following surgical treatment of pulmonary stenosis with intact ventricular septum undergoing the mapping and radiofrequency catheter ablation (RFCA) guided by three-dimensional (3D) electroanatomic system.Results:Low voltage area was identified at anterior wall of right ventricle, where the clinical VT exactly originated. The circuit of VT was surrounding the scar at anterior wall of right ventricle, with the isthmus located at the middle part of the anterior and lateral wall of right ventricle. Acute success was achieved and no recurrence was found during a follow-up period of 24 months. No complication occurred.Conclusions:The mechanism of VT following surgical treatment of pulmonary stenosis with intact ventricular septum can be both focal and reentrant. RFCA is feasible and effective for this kind of VT.
