ObjectivesThrough a retrospective analysis of the risk factors during the perioperative period of the hip replacement for the old flip patients, this paper aims to provide some suggestions for the treatment of the old hip fracture patients through the hip replacement in the future.MethodsThe retrospective research chose the medical history data of 320 old hip fracture patients above 60 years old who received hip replacement in Guangzhong Provincial Central Hospital and kept a correct record of the basic information, preoperative comorbidity, intraoperative situation, postoperative complication and so on. SPSS was employed to compare the data and conduct a statistical analysis.ResultsWere included in this study in elderly patients with hip fracture line hip replacement in 320 cases, single factor analysis results indicate that complications of central nervous system disease affected by anesthesia, postoperative use of TCM, the number and the effect of preoperative coexist disease complications of cardiovascular system diseases by age, intraoperative blood loss, postoperative use of TCM, the number and the effect of preoperative coexist disease complications in renal system disease by preoperative coexist disease the number, the influence of postoperative syndrome of traditional Chinese medicine, the complications of respiratory system diseases by traditional Chinese medicine (TCM) after use, the influence of the number of preoperative coexist disease, complications, electrolyte disorder by sex, operation time, postoperative use of TCM, the number and the effect of preoperative coexist disease complications of cardiovascular system diseases+electrolyte disorder by age, preoperative use Chinese medicine, Chinese traditional medicine to take following surgery, preoperative coexist disease the number, the influence of postoperative syndrome of TCM. Complications of urinary tract infection is not affected by 13 independent variables.Risk factors’analysis of central nervous system disease:in the case of anesthesia, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is smaller than 0 and the value of OR is smaller than 1, demonstrating negative correlation, anesthesia can effectively lower the risk of complication central nervous system disease. in the case of the numbers of coexistence, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is bigger than 0 and the value of OR is bigger than 1, demonstrating positive correlation, the more the coexistence before operation, the bigger the risk of emerging complication central nervous system disease will be; in the case of taking chinese traditional medicine after operation, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is smaller than 0 and the value of OR is smaller than 1, demonstrating negative correlation, taking Chinese traditional medicine after operation can effectively lower the risk of complication central nervous system disease. Risk factors’ analysis of cardiovascular system disease:in the case of intraoperative blood loss, the value of p is bigger than 0.05 so that discrepancy has no statistical significance, so there’s no co-relation between the postoperative cardiovascular system disease and intraoperative blood loss; in the case of age, the value of p is smaller than 0.05 so that discrepancy has statistical significance, regression coefficient is bigger than 0 and the value of OR is bigger than 1, demonstrating positive correlation, and the bigger the age is, the bigger of the risk of emerging complications cardiovascular system disease will be; in the case of the numbers of coexistence, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is bigger than 0 and the value of OR is bigger than 1, demonstrating positive correlation, the more the coexistence before operation, the bigger the risk of emerging complication cardiovascular system disease will be; in the case of taking chinese traditional medicine after operation, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is smaller than 0 and the value of OR is smaller than 1, demonstrating negative correlation, taking Chinese traditional medicine after operation can effectively lower the risk of complication cardiovascular system disease. Risk factors’analysis of kidney system disease:in the case of the numbers of coexistence, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is bigger than 0 and the value of OR is bigger than 1, demonstrating positive correlation, the more the coexistence before operation, the bigger the risk of emerging complication kidney system disease will be; in the case of TCM syndrome after operation, the value of P is bigger than 0.05 so that discrepancy has no statistical significance, therefore, there’s no correlation between the postoperative kidney system diseaseand postoperative TCM syndrome. Risk factors’analysis of respiratory system disease:in the case of the numbers of coexistence, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is bigger than 0 and the value of OR is bigger than 1, demonstrating positive correlation, the more the coexistence before operation, the bigger the risk of emerging complication respiratory system disease will be; in the case of taking chinese traditional medicine after operation, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is smaller than 0 and the value of OR is smaller than 1, demonstrating negative correlation, taking Chinese traditional medicine after operation can effectively lower the risk of complication respiratory system disease. Risk factors’analysis of electrolyte disturbance:in the case of gender, the value of p is smaller than 0.05 so that discrepancy has statistical significance, regression coefficient is bigger than 0 and the value of OR is bigger than 1, demonstrating positive correlation, and the man is the bigger of the risk of emerging complications electrolyte disturbance will be; in the case of operation time, the value of p is smaller than 0.05 so that discrepancy has statistical significance, regression coefficient is bigger than 0 and the value of OR is bigger than 1, demonstrating positive correlation, and the longer operation time is, the bigger of the risk of emerging complications electrolyte disturbance will be; in the case of taking chinese traditional medicine after operation, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is smaller than 0 and the value of OR is smaller than 1, demonstrating negative correlation, taking Chinese traditional medicine after operation can effectively lower the risk of complication electrolyte disturbance; in the case of the numbers of coexistence, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is bigger than 0 and the value of OR is bigger than 1, demonstrating positive correlation, the more the coexistence before operation, the bigger the risk of emerging complication electrolyte disturbance will be. Risk factors’analysis of cardiovascular system disease and electrolyte disturbance:in the case of age, the value of p is smaller than 0.05 so that discrepancy has statistical significance, regression coefficient is bigger than 0 and the value of OR is bigger than 1, demonstrating positive correlation, and the bigger the age is, the bigger of the risk of emerging complications cardiovascular system disease and electrolyte disturbance will be; in the case of taking Chinese traditional medicine before operation, the value of p is bigger than 0.05 so that discrepancy has no statistical significance, so there’s no co-relation between the postoperative cardiovascular system disease, electrolyte disturbance and preoperative traditional Chinese medicine taking situation; in the case of taking Chinese traditional medicine after operation, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is smaller than 0 and the value of OR is smaller than 1, demonstrating negative correlation, taking chinese traditional medicine after operation can effectively lower the risk of complication cardiovascular system disease and electrolyte disturbance; in the case of the numbers of coexistence, the value of p is smaller than 0.05 so that discrepancy has no statistical significance, regression coefficient is bigger than 0 and the value of OR is bigger than 1, demonstrating positive correlation, the more the coexistence before operation, the bigger the risk of emerging complication cardiovascular system disease and electrolyte disturbance will be; in the case of TCM syndrome after operation, the value of P is bigger than 0.05 so that discrepancy has no statistical significance, therefore, there’s no correlation between the postoperative cardiovascular system disease, electrolyte disturbance and postoperative TCM syndrome.Among the old hip fracture patients who receive hip replacement, fracture type, anesthesia style, operation style, intraoperative amount of bleeding, ASA classification of anesthesia risk, preoperative intake of TCM, TCM syndrome after operation are not related to the postoperative complications. The preoperative complications can influence the occurrence of most postoperative complications. The postoperative intake of TCM can contribute to the reduction of the postoperative complications. Gender will affect the occurrence of postoperative electrolyte disturbance; age will affect the occurrence of cardiovascular system complication and co-occurrence of cardiovascular system complication and electrolyte disturbance; anesthesia method will affect the occurrence of complication of postoperative central nervous system; operation time influence the occurrence of postoperative electrolyte disturbance complication.ConclusionPreoperative coexist disease is affected in elderly patients with hip fracture hip replacement risk factors for the occurrence of postoperative complications, in elderly patients with hip fracture hip replacement surgery should be deal with preoperative coexist disease, so as to reduce the risk of related to postoperative complications. Postoperative treatment to help Chinese medicine to reduce the occurrence of postoperative complications, suggest that in elderly patients with hip fracture hip replacement surgery can use traditional Chinese medicine treatment, so as to reduce the risk of related to postoperative complications.
