ObjectiveTo study the importance of the reconstruction of early ACL injury and the correlation between the ACL reconstruction time and articular cartilage damage in terms of the physique theory. The research can provide an objective basis for the treatment of early knee ACL injury and the recovery of knee-joint stability, and it can also provide the judgement basis for therapy and prognosis of diseases to some extent. We used arthroscopy to observe the degree of cartilage damage, then the clinical observation could tell us how long a patient got knee osteoarthritis after an ACL injury, that is knowed as the process from tendon damage to osteotrauma. This study remind us of paying attention to early treatment of tendon damage in the theoretical system of TCM and enrich the theory of TCM.MethodTo select 70 cases of unilateral ACL rupture in 2014 from The third affiliated Hospital of Guangzhou University of TCM and Guangzhou Orthopaedic Hospital. All patients have underwent autologous hamstring tendon ACL reconstruction under arthroscopy. According to the observation under arthroscopy,70 cases can be divied into two groups basis on cartilage injury. We mainly performed statistics and analysis of the time from initial injury to ACL reconstruction, and study the correlation between the time and cartilage injury.ResultThere are 70 cases of various age groups of patients with ACL injury (51 males and 19 females), that ranged from 18-year-old to 40-year-old. All patients underwent arthroscopic autologous hamstring tendon ACL reconstruction. According to the observation under arthroscopy,70 cases were divided into group A (no cartilage damage,37cases) and group B (cartilage damage,33 cases).Comparing the time from initial injury to ACL reconstruction of the two groups, the differences between group A and group B were statistically significant (P=0.002<0.05). The average injury time of Patients in group B was significantly higher than that in group A. The differences of times of sprain of the knee joint during the period from initial injury to surgery in these two groups were statistically significant (P=0.00<0.05). The times of sprain of the knee joint of patients in group B was significantly more than that in group A. The injury time (P=0.000<0.05,OR:0.725) and the times of repeated sprain (P=0.000<0.05, OR:0.615) were the risk factors for cartilage injury,70 patients were observed at different time of the incidence of cartilage damage. Within three months in which patients underwent surgery, cartilage injury was 7.89 percent, more than six months in patients undergoing surgery cartilage injury incidence as high as 94.44%. Different time segments constituting the incidence of cartilage damage compared, it is found within three months surgery patients with significant statistical difference compared with other periods.Preoperative Lysholm scores were significantly different contrast (P= 0.005<0.05), with a statistically significant, A group was significantly lower than in group B. The postoperative Lysholm score three month comparison was no significant difference (P=0.839>0.05). Two groups of patients were no severe complications after surgery.ConclusionTwo groups of patients postoperative Lysholm score two three-month comparison was no significant difference. Accept the ACL reconstruction within 3 months can significantly reduce the incidence of knee cartilage injury. Based on the "bones of both" that the treatment principle of traditional medicine. Found that ACL injury, they should actively receiving treatment. ACL injury should be actively treated, once ACL rupture needed to make an early ACL reconstruction.
