Exploration Of The Blood Loss And The Thrombin Change In Total Knee Arthroplasty Of Injection Of Tranexamic Acid

[objective] Exploration of the blood loss and the thrombin change in in total knee arthroplasty in the articular capuslue and the vein.[methods] the patients who got total knee atthroplasty from April 2014 to Feb.2015 were selected and divided to group A and group B.before the operation the patients of group A were injected tranexamic acid throw intravenous infusion, and then tranexamic acid were injected into the joint capsule during the operation.The group B were injected the equal normal saline before and during the operation.(the two groups were used the tourniquet during the operation,the blood loss was little,so there was no transfusion and the blood loss during the operation was ignored.)The patients go on the blood routine and clotting series at two days after the operation. Then they examine the ultrasonography of lower extremity after a week. The postoperative blood loss, number of transfusion, volume of drainage. hemoglobin.clotting series,thrombin time (TT),prothrombin time (PT), activated ptatrial thromboplastin time (APTT),D2 dimer were compared between the two groups.[Results] 1. The postoperative blood loss of group A was lesser than the group B, the group A is400.63 ±56.57ml, the group B is 666.24 ±71.49ml, there exsit statistical significance2. The volume of drainage of group A was lesser than the group B, the group A is 285.22 ± 34.50ml, the group B is 400.04 ± 50.92ml, there exsit statistical significance 。3. The hidden blood loss of group A was lesser than the group B, the group A is 123.23±23.65ml, the group B is 260.48±23.30ml, there exsit statistical significance。4.The hemoglobin of group A was lesser than the group B, the group A is 98.10 ± 13.86g/l, the group B is 88.65±10.43g/l, there exsit statistical significance。5.The prothrombin time (PT) of group A and group B were no statistical significance, the group A is 13.17±6.397/s, the group B is 11.3±2.14/s.6.The FIB of group A and group B were no statistical significance, the group A is 4.45 ± 0.82g/l, the group B is 4.49 ± 1.08g/l.7. The activated ptatrial thromboplastin time (APTT) of group A and group B were no statistical significance, the group A is 28.67±7.78/s, the group B is 29.8±5.53/s.8.The thrombin time (TT) of group A and group B were no statistical significance, the group A is 12.7±3.75/s, the group B is 13.7±3.67/s.9.The D2 dimer of group A and group B were no statistical significance ± the group A is 1.32±1.30ug/l, the group B is 1.31±1.18ug/l10. Lower extremity deep vein thrombosis was not found in the patients during one month after the operation.[conclusion] The blood loss in total knee arthroplasty after injection of tranexamic acid,was reduced and Lower extremity deep vein thrombosis were no statistical significance.
