Objective:To discuss the application value of serum cystatin C in the diagnosis of early Henoch-Schoenlein purpura renal damage,and the relationship of CysC and Children Henoch-Schoenlein purpura renal pathology,and the relationship between the severity of some allergic purpura in children with kidney damage detection index between clinical symptoms were related to exploration.Methods:301 cases of Henoch-Schoenlein purpura children were choosen in our Hospital in recent years, then detected the Cys C,24 hours urinary protein quantitative, serum creatinine (SCr) concentrations, and calculated the endogenous creatinine clearance (CCr).The children were divided into Henoch-Schoenlein purpura group and Henoch-Schoenlein purpura nephritis group, analyzing the difference of CCr, SCr and Cys C level in the two groups. The Henoch-Schoenlein purpura nephritis group divided into groups by albuminuria level and renal pathology,then analysis the difference of CCr, SCr and Cys C level between each group and the difference of Cys C level in renal insufficiency grade differences in HSPN group.The 301 cases of children with HSP group according to clinical symptoms were further divided into the relevant section of the symptoms, no joint symptoms group; group had gastrointestinal symptoms, no gastrointestinal symptoms group; then compared between the two groups of CCr, SCr and Cys C levels with indifference, and compare the relevant section at same time of the symptoms, gastrointestinal symptoms and gross hematuria between no joint symptoms, gastrointestinal symptoms, and the symptoms of gross hematuria group CCr, SCr and Cys C levels with indifference.Results:The difference of SCr between Henoch-Schoenlein purpura group and Henoch-Schoenlein purpura nephritis group had no statistical significance (P>0.05),and the difference of CCr, Cys C has statistical significance (P<0.05);Children with HSPN pathology classification, CCr, SCr, there was no statistical significance (P> 0.05) difference comparison, Cys C comparative difference was statistically significant (P< 0.01); There was significant difference of Cys C between the renal inadequacy level 1,2,3 of Henoch-Schoenlein purpura nephritis group (P<0.01), and there was no statistical significance (P> 0.05) difference of SCr. The difference of CCr. SCr and Cys C between the proteinuria normal, mild, moderate, severe groups of Henoch-Schoenlein purpura nephritis group had no statistical significance (P>0.05). There was no difference of CCr, SCr and CysC levels between with joint symptoms group and no joint symptoms group in 301 cases of HSP children. And the gastrointestinal symptoms group, had no significant difference with the non-gastrointestinal symptoms set of CCr, SCr and Cys C levels. Besides, the difference of CCr, SCr and Cys C levels between the relevant section at the same time of the symptoms, gastrointestinal symptoms and gross hematuria between no joint symptoms, gastrointestinal symptoms, and the symptoms of gross hematuria group had no statistical significance (P>0.05).Conclusions:The level of serum Cys C should be one of the evaluation indexes of Henoch-Schoenlein purpura nephritis renal damage,also could be one of the sensitive indexes of Henoch-Schoenlein purpura nephritis lesion extent. There was no significant correlation between the clinical Gastrointestinal symptoms, joint symptoms, gross hematuria symptoms and the kidney damage detection index in children with allergic purpura.
