Objective: Transient ischemic attack(transient ischemic attack, TIA) is a risk factor for stroke, the study found that about half before the onset of cerebral apoplexy patients have a history of TIA, so rapid clinical risk stratification in patients with TIA, and actively to control risk factors intervention, can effectively reduce the TIA patients progress for cerebral infarction.This research through carries on the summary and the analysis of clinical data of patients with TIA, explore serum homocysteine(HCY, homocysteic acid), plasma fibrinogen(fibrinogen, FIB), D-dimer and ABCD3-i score predicts transient ischemic attack(TIA) patients with early development of the value of the cerebral infarction.Methods:This study intends to select 2014-1-2015-1 in hebei cangzhou combine traditional Chinese and western medicine hospital neurology diagnosis of TIA patients as the research object, all of the patients in within 2 days(48 hours) of the clinical examination and inspection. D-dimer detection: the Japanese SYSMEX automatic thrombosis/hemostasis analyzer;HCY detection: the Hitachi 7600-7600 automatic analyzer, latex turbidimetric method;FIB detection: the French at full automatic blood coagulation instrument thrombin turbidimetric method;Head mri: Germany Siemens 3.0 T.(reference: D-dimer<0.5mg/l; blood homocysteine 4~15.4umol/l, fibrinogen 2~4 g/l).Statistics at least 100 patients with TIA were a risk factor for ischemic cerebrovascular disease related(HCY, FIB, D-dimer level) and ABCD3-I ratings, and statistics the incidence of cerebral infarction within seven days, the application SPSS19.0 software for statistical analysis.Results: In 110 patients with TIA, 66 men, average age(49.56±12.4 mm), 44 women, average age(56.87±11.42), the clinical symptoms of par TIAl body weakness, dizziness, physical activity and speech fluent owed, unilateral limb involuntary jitter, etc. dizziness.We divide the 110 patients into two group:the cerebral infarction-occuring group and the non-cerebral infarction—occuring group.By means of the two groups of comparison and analysis of data,we found that serum level of homocysteine(HCY) and fibrinogen(FIB) was statistically difference between the two groups(P<0.05). Disease patients with 7 days there were 15 cases(13.6%), cerebral infarction, grouped by ABCD3-I score, low-risk groups with no cerebral infarction, moderate group has 6 cases of cerebral infarction, high-risk groups is 9 cases of cerebral infarction. In ABCD3-I score in cerebral infarction group and cerebral infarction in comparison and found that 140/90 mm Hg or higher blood pressure, diabetes, clinical symptom duration or 10 min, brain imaging(DWI sequence) can find abnormal high signal and ipsilateral carotid stenosis >50% was statistically difference(P<0.05).Conclusion:Transient ischemic attack patients serum homocysteine(HCY) and fibrinogen(FIB) of the cerebral infarction occurred significantly higher risk, ABCD3-I score in the high-risk group is bigger than the risk of moderate-risk group and no low-risk group took place the cerebral infarction. In ABCD3-I score, blood pressure is 140/90 mm Hg or higher, with diabetes, clinical symptom duration or 10 min, brain imaging(DWI sequence) can find abnormal high signal and ipsilateral carotid stenosis >50% cerebral infarction occurred more often.
