ObjectiveTo compare the efficacy of the percutaneous nephrolithotomy(PCNL) in the treatment of upper urinary calculi in different access.Methods719 cases underwent PCNL from October 2009 to June 2013 were reviewed retrospectively. the cases were divided those cases into four groups:staghorn renal calculi group, multiple renal stones group, smple renal calculi combined with ureteral calculi group,simple renal calculi or ureteral calculi group. The differences in the first period calculus completely clearance rate,operation time and intraoperative and postoperative complications in different access.ResultFor the staghorn calculi group, the first period calculus completely cle arance rate was respectively 86.9% and 70.9%(P=0.025), operation ti- me was respectively (113.6±44.2) and (131.1±45.5) min (P=0.021). Th e multiple stones group first period calculus completely clearance rate was 86.6% and 75.8%, respectively (P=0.033), operation time was(94.5±39. 5) and (115.5±47.8)min (P=0.012). For renal calculi combined with uret eral calculi group the first period calculus completely clearance rate was 100% and 94.2%(P=0.357), respectively, operation time was (81.3±31. 2)和(104.5±39.3) min (P=0.019), respectively.The simple renal calculi or ureteral calculi group first period calculus completely clearance rate was 1 00% and 94.4%, respectively (P=0.504), operation time was (74.3±33.9) and (78.3±36.5) min (P=0.616) The puncture success rate was 100% to all the cases.ConclusionFor staghorn calculi and multiple stones through upper pole access; the firsr period calculus completely clearance rate is higher, operation time is shorter. so,we suggest to treat the staghorn calculi and multiple stones through upper pole access.
