Multi-Slice Spiralct And Endoscopic Ultrasonography In Preoperative Gastric Cancer Tnm Linical Significance Of Staging

Objective Investigate the value of multi-slice spiral CT and endoscopic ultrasonography in preoperative TNM staging of gastric cancer, provides the reference for the clinical diagnosis and treatment of gastric cancer.Method Selected from 2012 July to 2015 February,gastrointestinal surgery, Tumor Hospital Affiliated to Guangxi Medical University.146 cases of gastric cancer patients after operation treatment as the study object, before surgery did not do any treatment. Application of multislice spiral CT in patients with gastric cancer, endoscopic ultrasonography in preoperative TNM staging were examined in parallel, with the Postoperative pathology as the gold standard, comparison,observation both in gastric cancer with preoperative TNM staging accuracy and it’s value.Results 1. Multi-slice spiral CT in T staging accuracy rate was 75.34%, the accuracy rate of Tl to T4were:54.55%,66.67%,84.84%,76.47%; the 12.33% (18/146) appear excessive andinadequate staging staging. The N staging accuracy rate was 76.71%, the accuracy rate of N-,N+ were:81.82%,74.51%; There were 5.48%(8/146) of the excessive,17.81%(26/146) of the stage was insufficient.The accuracy of M staging is as follows:97.52%, the accuracy of M1 and MO are 95.20%,84%,2.05%(3/146) and 2.74%(4/146) are staging problems.2. Endoscopic ultrasonography for T staging accuracy rate was 72.60%, the accuracy rate of T1～T4 were 63.33%,75%,78.78%,64.71%; the 13.70%(20/ 146) appear excessive andinadequate staging staging. The N staging accuracy rate was 67.12%, the accuracy rate of N-,N+were:77.27%,62.75%; There were 6.84%(10/146) of the excessive,26.03%(38/146) of the stage was insufficient.The accuracy of M staging is as follows:91.74%, the accuracy of M1 and M0 are 83.56%,56%,6.85%(10/146) and 29.59%(14/146) are staging problems.3.By multi-slice spiral CT and endoscopic ultrasonography in preoperative TNM staging of gastric cancer the results found in T1,T4, N total,M1 were significant difference between the total accuracy rate (P<0.05).5.The accuracy of multislice spiral CTcombined with endoscopic ultrasonography in diagnosis of T staging was 91.78(134/146), the accuracy rate of diagnosis and staging of N was 91.10%(133/146),the accuracy of M staging was 96.57%(141/146).Conclusion 1. EUS has a high accuracy for the depth of invasion of early gastric cancer, and MSCT has a guiding significance for the surgical resection of gastric cancer.2.MSCT on the N staging accuracy is relatively high, and has important significance to guide the intraoperative lymph node dissection.3.For gastric cancer distant metastasis, EUS judgment ability is limited.4.MSCT combined with EUS can significantly improve the accuracy of the preoperative staging, provide a more reliable basis for the diagnosis and...
