Object: Compare the difference of brain iron deposition of regions of interest(ROIs) in the Parkinson’s disease(PD) group and normal control(NC) group with susceptibility-weighted imaging(SWI) at 3.0 Tesla to evaluate the clinical value of SWI as a way to diagnose and monitor the state of illness of Parkinson’s disease.Methods: All patients were recruited from The First Affiliated Hospital of Guangxi Medical University diagnosed Parkinson’s disease.There were 41 patients with PD(19men,22 women) aged 58.98±9.77 years,who had a mean disease duration of 33.39±22.721 months,and 42 age- and sex-matched volunteers(19 men and 23 women)as a normal control group(NC),whose mean age were 58.60±10.378 years. Conventional brain MRI and SWI images were examined. The phase value of the bilateral substantia nigra(SN),red nucleus(RN),head of caudate nucleus(CN),and putamen(PUT)were evaluated in the corrected phase image. Phase values were obtained from the formula as follows: f(x)=-x×π/4096 and analyzed Statistically.Results:1.The NC group:(1)In the NC group statistical difference were found between the Phase value of SN and RN、CN 、PUT, P<0.05,and the Phase value of SN lower than the rest of ROIs, so were between the Phase value of front、medical and posterior of bilateral SN and PUT.No statistical difference were found between RN、CN and PUT.(2)No statistical difference were found in RN、CN and PUT between different genders, P>0.05.(3)Statistical difference that the left Phase value of SN and RN were lower than the right ones were found,P<0.05,so were between lateral and medial of bilateral SN indicating the Phase value of lateral lower than the other side. But no statistical differences were found between left and right PUT and CN.(4)Statistical difference were found between the anterior、intermediate and posterior in SN,P <0.05,so were between the anterior、intermediate and posterior in PUT,indicating the Phase value of the anterior lower than the Phase value of others in SN and the Phase value of the posterior higher than the Phase value of others in PUT whose P was 0.002. 2.The PD group:(1)No statistical difference were found between the symptomatic side and the asymptomatic side of ROIs in the PD group.(2)Statistical difference were found between initial stage(HoehnYahr≤2.5) and advanced stage(Hoehn-Yahr>2.5) of PUT and CN,p=0.001,and the phase value of advanced stage was lower.(3)No statistical difference of the phase value of the symptomatic side and the asymptomatic side in ROIs between duration≤36 months Group and >36 months Group were found, p>0.05.so were the mean value of bilateral ROIs.(4)There was no correlation between the phase value of the symptomatic side and the asymptomatic side of ROIs、duration、total scores of UPDRS 、motor scores of UPDRS were found, p>0.05. 3.Comparison between the normal control group and the PD group:(1)Statistical difference between the control group and the PD group were found in SN and PUT, p=0.015、0.005,but not in RN and CN,P=0.105、0.305.(2)Statistical difference in SN and PUT of the asymptomatic side between the PD group and the normal control group were found, p = 0.003、0.004,so were in the medical of SN at the asymptomatic side and the symptomatic side, p<0.05,and the phase value in medical SN of asymptomatic side reduced more apparently,p=0.035.(3)Statistical difference in the anterior、and posterior of PUT were found between the PD group and the normal control group,p<0.05,and the phase value in the PD group is lower.Conclusion: 1. In physiological conditions, a leftward asymmetry of iron content in brain has been observed. There has been a lot of pathological accumulation of iron in initial stage of PD in bilateral SN which play a key role in offering DA and SNc should be responsible for the accumulation mainly and the asymptomatic side is more remarkable. With the progression of PD, it refers to PUT and the asymptomatic side is more significant. 2. Susceptibility weighted imaging was sensitive to iron.It can be used to monitor the progression of PD, as a simple and noninvasive mean.
