ObjectiveThe aim of this study was to know the discrepancy of hepatitis virus infection between B cell chronic lymphoproliferative disorders (B-CLPD) and aggressive B non-Hodgkin’s lymphoma (B-NHL). To analyse the infection rate of subtypes of B-CLPD and evaluate the association of subtypes of B-CLPD and hepatitis virus infection.Methods733 B-CLPD patients and 123 aggressive B-NHL patients from January 1994 to January 2014 with integrated clinical information were retrospective investigated in this study. Compare the difference of Hepatitis virus infection between the two groups and analyse the infection rate of hepatitis virus and the association of different subtypes of B-CLPD.ResultsThe anti-hepatitis A virus antibody (HAV-Ab) and anti-hepatitis C virus antibody (HCV-Ab) positive rate was 1.1% and 1.9% of the 733 B-CLPD patients, which had no significantly difference compared with the 132 aggressive B-NHL (P>0.05). The B-CLPD patients had a lower prevalence of positive HBs-Ag compared with the aggressive B-NHL patients (7.9% vs 16.5%,P=0.001).The coexpression of HBs-Ag,HBe-Ag and anti-HBc-Ab was 1.1% in B-CLPD patients,which was significantly lower than that of the aggressive ones(1.1% vs 6.0%,P=0.000).The coexpression of HBs-Ag,HBe-Ab and anti-HBc-Ab had no significantly difference between the two groups(P=0.315).The histological types of 733 B-CLPD patients:The chronic lymphocytic leukemia(CLL) in 279/733 (38.1%)patients, Waldenstrom macroglobulinemia (WM) in 119/733 (16.2%)patients, follicular lymphoma (FL)in 74/733(10.1%) patients, B cell lymphoproliferative disorders,unclassified (B-LPDU) in 66 /733 (9.0%)patients, splenic marginal zone lymphoma (SMZL)in 48/733 (6.5%) patients, hairy cell leukemia(HCL) in 33/733 (4.5%)patients,nodal marginal zone B-cell lymphoma (NMZL) in 27/733 (3.7%) patients,extranodal marginal zone B-cell lymphoma of mucosa-associated tissue lymphoma (MALT) in 15/733 (2.0%) patients.B-cell prolymphocytic leukaemia (B-PLL) in 6 (0.8%) patients,The suspected SMZL in 40/733(5.5%) patients and the suspected mantle cell lymphoma(MCL) in 26/733(3.6%)patients.The positive rates of anti-HAV-Ab and anti-HCV-Ab were no significantly different between different subtype B-CLPD patients (P>0.05). HBsAg positive rate was 18.8% in the SMZL patients, which was significantly higher than other B-CLPD subtypes (P=0.004) and other MZL patients.(P=0.005).The coexpression of HBsAg,HBeAg and anti-HBcAb had no significantly difference between the different B-CLPD subtypes groups. However the coexpression of HBsAg,HBeAb and anti-HBcAb was significantly higher in the SMZL group than other B-CLPD subtype groups(16.6% vs 4.7%,P=0.000).Of the 284(38.7%)HBV-infected patients, the HBsAg positive rate was higher in the SMZL group than that of the other B-CLPD patients(34.6% vs 19%,P=0.06). The coexpression of HBsAg, HBeAg and anti-HBcAb had no significantly difference between different B-CLPD subtypes. However, compared with the other B-CLPD patients, the coexpression of HBsAg,HBeAb and anti-HBcAb was higher in the SMZL group(30.8% vs 12.1%,P=0.009).ConclusionsThe infection rate of HAV and HCV was no significantly different between B-CLPD and aggressive B-NHL group as well as different B-CLPD subtypes. The prevalence rate of HBV was higher in the SMZL group than other B-CLPD groups suggest that HBV infection may play an etiologic role in SMZL.
