The Clinical Research Of Xinmailong Injection On Cardiac Function And Extravascular Lung Water Of Acute Left Ventricular Failure

Background:Heart failure, mainly acute left heart failure(ALHF), is the disease with a high prevalence morbidity. It has become the main cause of resident patients over 65 years old with a poor prognosis. In the treatment on ALHF, the western medicine on reducing pulmonary edema and ameliorating cardiac function is not ideally efficient.Objective:The purpose of this study was to study the effect of Xinmailong injection on cardiac function and extravascular lung water(EVLW) of ALHF.Method:The controlled clinical study was performed on the basis of evidence based medicine. From July 2014 to February 2015 in TCM Hospital of Guangdong Province, there were 50 resident patients, who admitted to acute left ventricular failure, included in the study. There were two different groups they divided in, including the comparative group(30 cases) and the therapy group(20 cases). Each group was based on routine treatment of western medicine. In addition, the therapy group received Xinmailong injection therapy, while the comparative group was given a blank control. After 5 days of remedy, B-lines, left ventricular ejection fraction(LVEF), left ventricular fractional shortening(FS), stroke volume(CO), stroke volume(SV), high sensitivity cardiac troponin I(hs-cTnI), cardiac enzymes, B-type natriuretic peptide(BNP) of two groups were compared. Minnesota score and NYHA cardiac functional grading were compared in 30th days.Results:There were 50 cases accomplished, including 20 cases of therapy group(40%), 30 cases of the comparative group(60%). Compared two groups of baseline, as sex, age, concomitant disease, etc, they have no obvious difference (P>0.05). After the 5-days-treatment, B-lines, NYHA cardiac functional grading of the therapy group were demoted more than the comparative group(P<0.05). The LVEF improved more saliently(P<0.05). The score of Minnesota living with heart failure of 30th day of the therapy group were reduced more than the comparative group(P<0.05). Before and after the remedy, the liver function indexes, including alanine aminotransferase (ALT), aspartate aminotransferase (AST) of two groups have no obvious differences(P>0.05).Conclusion:Xinmailong injection can decrease the injury of myocardial cell in patients with ALHF, strengthen myocardial contractility, ameliorate cardiac function, demote extravascular lung water, thereby lessen symptoms, ameliorate the prognosis in a short time.
