Ischemia-reperfusion injury of liver is a common phenomenon of pathophysiology in the clinical.The most secondary to liver transplantation or complex liver surgery.The effect of ischemiareperfusion injury of liver to the body and postoperative liver regeneration of animal experiments most focused on the liver pathology model of mice and rats.But, reported rarely as the model of ischemiareperfusion injury in miniature pigs of similar to human on the physiological structure and function.So, in this study,used the technology of laparoscopic minimally invasive surgery to set the model of injury of liver in miniature pigs.Explore the impact of miniature pigs physiological function and liver regeneration at different ischemia time and partial nephrectomy damage in the laparoscopy. Analysis the change of the liver microenvironment and the ability of regeneration after the injure. It provided the basic data for the build a suitable model of ischemia-reperfusion injury in the liver of miniature pigs.In this study,choice 25 healthy miniature pigs, divided into 5 groups randomly, the control group(A group), the left liver resection group(B group), the right half liver ischemia 30 min and left liver resection group(C group), the right half liver ischemia 60 min and left liver resection group(D group), the right half liver ischemia 90 min and left liver resection group(E group). During the experiment, set up a small pig liver ischemia and injury model by the same means of anesthesia, pneumoperitoneum andoperation pathways.During the operation, entire continuous detection for important physiological indexes.Each team take the venous blood at preoperative, postoperative immediately, postoperative 1d,3d,7d,14 d and 30 d. Detecte the change of total protein, aspertate aminotransferase, r-glutamine transferase, total bilirubin, hepatocyte growth factor and Transforming growth factor. Liver biopsy at preoperative, immediately after ischemia, postoperative immediately postoperative 1d,3d,7d,and 30 d.Used HE dyeing observe the change of histopathological,at the same time,detect the change of Proliferating cell nu clear antigen(pcna), Toll-like receptors, Nuclear factor-κB, Tumor necrosis factor-α, Interleukin – 6.Through the experiment, each operation was successfully completed. The physiologicalindexes of each group in the operation process of miniature pigs have different degrees ofchange, such as increased heart rate, breathing at the end of the partial pressure of carbon dioxide increased, decreased body temperature, in addition to the physiologicalright liver ischemia 90 min with left hepatectomy group within the individual animal refers to the large fluctuation range, the other groups were not beyond the normal range of tolerance in the postoperative, and soon returned to normal.The effect of surgery for the function of miniature pigs liver show that, compare with the preoperative, the change of Serum AST indicate statistical extremely significance(P<0.05) at postoperative 1d,3d.After ischemia and postoperative 1d, Serious degeneration of liver cells transgender and flammatory infiltration and inflammatory infiltrates. postoperative 7d liver cell proliferation significantly. Postoperative groups Hepatocyte growth factor increased first, then decreased in serum.At postoperative 1d achieve the highest,then reduced gradually. Transforming growth factor After assumes tendency which first reduces then elevates.Indicate statistical significance(0.01<P<0.05) at postoperative 3d and 14 d. Hepatocellular PCNA expression level peak after postoperative 1d. Then with the time reduced gradually. The expression of Toll-like receptors, Nuclear factor-κB Significantly increase at the early of Ischemia-reperfusion injury. Significantly increase at the postoperative immediately, postoperative 1d. indicate statistical extremely significance(P<0,01).then decrease at postoperative 3d, Indicate statistical significance(0.01<P<0.05),other team Indicate statistical not significance(P>0.05). The expression of Tumor necrosis factor-α and Interleukin – 6 increased first, then decreased. At postoperative 1d rapid rise to the peak,then at the postoperative 3d,7d reduce gradually,but,the range of Interleukin – 6 smaller.The results show that the following conclusions:1.The application of laparoscopic operation successfully established liver pigs at different ischemic time combined with partial resection of the damage model, each operation was successfully completed, proved that the experimental method is feasible.2.In the operation process of miniature pig physiological indexes of respiratory and circulatory syst-em of monitoring and evaluation, and through the detection and analysis of liver tissue and serum after operation, proved that the experimental method is safe.3.According to the analysis of the testing results of comprehensive comparison, and ultimately dete-rmine the right liver ischemia 60 min with left liver injury cut into moderate,small pig liver ischemia and injury induced obvious liver regeneration.
