Part I Effect and possible mechanism after 5-aminolevulinic Acid Photodynamic Therapy in chicken combObjective: To investigate the effect and possible mechanism of topical 5-aminolevulinic Acid(5-ALA)in combined with 630 nm red laser light photodynamic therapy(PDT) on vascular lesion of cock comb as the model of port wine stains(PWS).Also, to explore the feasibility of topical ALA-PDT on PWS. Methods: A total of 80 chickens were divided into 10 groups randomly, including control group, topical ALA cream group, irradiation group which were received 630 nm red laser light irradiation and ALA-PDT group received irradiation after lesionsed of topical ALA cream, with 8 in each group. The morphology of combs was observed, and tissue specimens, on the 14 th, 28 th day, were reseeted from the combs for the analysis of histological changes and the decrease rate of the number of capillary vessels was calculated. Besides, TUNEL staining was conducted to observe the condition of capillary endothelium apoptosis. Results: The combs in ALA-PDT group showed blanching and decrease of blood vessel amount, waning vascular caliber and partial capillary endothelium apoptosis were found. The decrease rate of the number of capillary vessels(14d)in ALA-PDT group(33.53%±4.89%, 52.02% ±2.77%,67.48%±5.58%, 88.96%± 2.47%)was significantly increased than in other groups(all P<0.01).The apoptosis index(14d) in ALA-PDT group(63.44± 1.09,88.50 ±6.11,94.32±3.67,113.76±10.57)was significantly increased than others(all P<0.01).The apoptosis obviously in depth, in ALA-PDT group was as follows:201.19±0.33μm,266.15±1.02μm,546.09 ±2.45μm and 766.37±1.08 μm, was similar between each other(all P<0.01). Conclusion: Topical ALA-PDT is proved to have effective damage to cutaneous micrangium of cock comb, the depth and extent of damage was related to the dose of light irradiation, one of the mechanism of photodynamic therapy maybe to induce apoptosis of endothelium of capillary. Topical ALA-PDT is expected to become an effective treatment applicable to the clinical treatment of port-wine stains.Part II A research of damaging effect after aminolevulinic Acid combined with intense pulsed light in chicken combObjective: To investigate the damage effect and possible mechanism after 5-ALA combined with intense pulsed light(IPL)on vascular lesion of cock comb as the model of port wine stains(PWS). Methods: A total of 16 chickens were divided into 4 groups, with 4 in each group. Group A was the control group, Group B were treaded by IPL irradiation for one session, Group C were treaded by IPL irradiation after external ALA cream and Group D were treaded by 630 nm red laser light irradiation(125J/CM2)after external ALA cream. On the 14th、28th day after the irradiation,the morphology and tissue specimens of combs was observed, the depth of damage was measured and the decrease rate of the number of capillary vessels was calculated. Besides, TUNEL staining was conducted to observe the condition of capillary endothelium apoptosis. Results: The combs in groups B 、C 、D showed blanching. According to histological observation, groups B、C 、D shows decreasing number of vessels and waning vascular caliber, with decrease rate of the number of capillary vessels as 67.92%±1.79%、89.17%±2.33%、61.58%±1.03%,than in the control group(all P<0.01). And the depth of damage of Group B、C and D was 235.25± 3.27μm、373.77± 4.32μm、199.84±1.46μm. Partial capillary endothelium apoptosis were found in group C、D, with the apoptosis index of 101.76±2.07、90.28±1.34, significantly lower in group A(1.09±0.88)and group B(4.37 ±0.98).The occurrence of apoptosis in depth in Group C、D were 759.19±0.33μm、438.25±0.68μm. Conclusion:Topical ALA in combination with IPL,seems superior to that of IPL alone, can be applicable to the clinical treatment of PWS.However,one of the mechanism of maybe to induce apoptosis of endothelium of. Capillary.
