Objective: To discuss the value of 128 multislice-spiral CT perfusion imaging(CTPI) in preoperative grading of gliomas. Methods : 64 patients with gliomas were divided into low-grade group(grade I- II) and high-grade group(grade Ⅲ- IV), CT perfusion scans were performed respectively, measure CBF, CBV, MTT, PMB of the tumor lesions and the contralateral normal brain tissue. Calculate rCBF rCBV and rMTT, rPMB, in addition. Analysis the statistical data of two groups with SPSS18.0 statistical software. Results: In these 64 case of glioma patients, CBF、CBV、PMB value of tumor area were significantly higher than the contralateral normal brain tissue regions(P <0.05); Between the two groups, CBF, CBV, PMB,MTT value of normal brain tissue area, the difference was not statistically significant(P> 0.05); CBF, CBV, PMB, r CBF, rCBV, rPMB value of high-grade-guoup(HGG) tumor area were significantly higher than the low-grade-group(LGG)(P <0.05); The difference of the MTT, rMTT value was not statistically significant between the two groups(P> 0.05); We can judge the glioma was HGG, when CBF>51.83、CBV>7.59、PMB>17.5、rCBF>3.20、rCBV>3.15、rPMB>6.57. Conclusions: 128 Multi-slice spiral CT perfusion imaging has a high clinical application value in glioma preoperative grading.Compared with the other CT perfusion imaging parameters, rCBV and PMB have higher sensitivity and specific degrees in the preoperative grading of gliomas.
