Apply Spino-Pelvic Sagittal Parameters To Evaluate The Correlation Of Reset Level And Postoprative Efficacy In Lumbar Spondyloysis

Purpose:To evaluate the correlation of reset level and the clinical efficacy in lumbar spondylolysis by applying sagittal parameters, therefore, acquire patients’ individual evaluation of the reset methods before the operation.Method: Following-up the patients of isthmic lumbar spondylolysis who were treated by surgical slippage resetted in Huaihua No.1 People’s Hospital from 2008 to 2014. To assess the patients, postoperative quality of life by JOA assessment, acquiring patients’ surgical treatment period, spino-pelvic full-length side X-ray, spino- pelvic sagittal parameters, such as pelvic incidence( PI), pelvic tilt(PT), sacral slope(SS), lumbar lordosis(LL),thoracic kyphosis(TK). According to the PI, the patients were divided into three groups. Group A PI<45 °,group B 45 °≤ PI≤ 60°,group C PI >60°,the differences of the three groups, curative effects were identified by Q test analysis, the correlation among the spino- pelvic sagittal parameters and postoperative period by Pearson analysis.Results:1、 Significant differences exist among three groups, were showed below : A(PI<45 °)>B(45 °≤ PI≤ 60°)>C( PI >60°)(P<0.01).2、Pearson correlation analysis showed that significant correlation among period and PI、LL、SS、PT. There was smaller correlation between TK and period, but significant correlation between PI and LL. Conclusion :1、 Patients with isthmic lumbar spondylolysis resettd by surgery can improve patients’ quality of life, But there are differences in the clinical efficacy among different sizes of PI after reset by surgery. By surgical reposition, clinical curative effect of smaller PI is superior than higher PI. We found that the efficacy were worse if the reset level were bigger of the higher PI.2、There are significant individual differences in spino-pelvic sagittal parameters,the living quality of patients and parameters ratio distribution of spino-pelvis in sagittal is significant correlation, Further research is needed to find appropriate collocation ratio.3、Surgery treatment for isthmic lumbar spondylolysis to reset, the value of PI should be fully considered. According to different size of PI to take a different degree of reset,smaller PI reset as far as possible, patients with higher PI could choose part of reset, reset level should be researched in the future.
