| Objective : To understand the distribution status of health personnel in the basic medical institutions of Guizhou, explore the status quo of supply and demand, and the influence factors of the health human resources in the medical institutions of Guizhou Province, explore the incentives of health professionals of Guizhou primary health system, and provide scientific basis for formulating incentive system.Methods: Refer to the Guizhou province health statistics yearbook and Guizhou province health statisticsin 2010,collect the data of health human resources and health service utilization in different areas of Guizhou province, analyze the distribution status of health personnel in the basic medical institutions of Guizhou. The paper adopts health service demand approach to measure and calculate the health human resources demand in Guizhou province while comparing with the existing health human resources. It also uses multiple linear regressions to analyze the factors that influence the number of health personnel.Random draw three sample countie according to the level of economic development, each county draw 25 community health service center(station)/ health clinics in towns and townships, each community health service center(station)/ health clinics in towns and townships extract 1-2 medical personnel as the object of investigation involved in medical personnel questionnaire survey, using the fill in the questionnaire survey. Refer to the related literature at home and abroad and Consult relevant experts, design questionnaire in combination with the practical situation of guizhou province. Research content include health social demographic characteristics, the satisfaction of health personnel about the motivating factors and overall satisfaction of health policy, etc. Social demographic characteristics include age, gender, nationality, marital status, education level, income status, professional and departments, etc. Incentive satisfaction include income and treatment, working time and pressure, medical training and continuing education situation, professional identity and job satisfaction, 20 variables, eight variables for the health policy overall satisfaction. Data was applied to study the incentives with factor analysis, and analyze the incentives which affect the health policy satisfaction with multiple linear regression. It also uses multiple linear regressions to analyze the relation between the incentives and satisfactions. Results: The required number of practicing doctors and assistant practicing doctors in Guizhou Province is 41,117, and the current number of doctors is 43,389, while that of Guiyang, Liupanshui, the Southeast of Guizhou and Qiannan is more than what is required. The number of registered nurses, pharmacist and technicians is less than the required number. The existing administrative staff number is able to satisfy the needs. The number of practicing doctors and assistant practicing doctors can meet the needs in hospitals, health centers and community health service centers. The number of registered nurses is able to satisfy the needs of hospitals, but their number for health centers or community health service centers is apparently inadequate. Practitioners’ ratio was higher in male than in female, health personnel degree is low, mainly in the technical secondary school and below, the title is low, mainly concentrated in the heads of/assistant and below, the medical configuration is obviously unreasonable, nursing staff shortage is obviously.Multiple linear regression analysis shows that the influence factors upon the number of health workers in hospitals are, in order, general income, medical expenses, number of beds, personnel outlays, equipment valued over 10,000 Yuan and financial subsidy, while those in health centers and community health service centers are, in order, business occupancy, in-patient numbers, average length of hospitalization and the utilization rate of hospital beds.Exploring factor analysis show that, incentives of health professionals in Guizhou primary health system include social identity, management and work autonomy, working pressure, income, training and development, work of the sense of honor, interpersonal relationship and living conditions. Multiple linear regression analysis shows that the incentives upon the effects of health policy satisfaction are, in order, income, management and work autonomy, training and development, interpersonal relationship, social identity, work of the sense of honor, living conditions.Multiple linear regression analysis in different areas shows that the influence factors in 1st class area, in order, management and work autonomy, pride, living conditions, income, social identity, training and development, relationship; 2nd class area, the influence factor is management and work autonomy only; 3rd class area, in order, income, management and job autonomy, relationship, living conditions.Conclusion: There is a large gap between the areas of health human resources distribution, hospitals and basic medical institutions in Guizhou Province, so the government and related functional departments should take corresponding measures to bridge the gap, such as improve primary health personnel wages and living conditions, increase health personnel job autonomy, increase training efforts, certain health personnel work performance, etc. |
