0bjective:To investigate the effects of simultaneous multi-level surgery for moderate to severe obstructive sleep apnea hypopnea syndrome(OSAHS),And curative effect evaluation method will be discussed in this paper.Methods:A retrospective analysis was made on surgical cases of one hundred thirty seven patients with moderate to severe OSAHS diagnosed by polysomnography(PSG).They were divided into multi-level group(n=95)and UPPP group(n=42).multi-level group underwent simultaneous multi-level surgery which was based on modified UPPP,and UPPP group underwent only modified UPPP treatment. The postoperative complications were compared,as well as the related indicators of PSG,Calgary sleep apnea quality of life index(SAQLI), Epworth sleepiness scale(ESS) and Snore scales(SS) before operation and 6-month after operation.Results:One patient in the multi-level group had breathing difficulty and was rescued by timely tracheotomy. There was no significant difference in Visual analogue scale(VAS)between two groups(P>0.05),as well as in incidence of wound blooding and temporal velopharyngeal insufficiency.While the incidence of pharyngeal foreign body sensation and tongue numbness were higher in multi-level group than that in UPPP group(P<0.05).AHI,LSa O2,TS90%,the total score and four dimensions(including daily activities, social interaction, emotion and symptom) scores in SAQLI,ESS score,SS score were significantly improved after operation in multi-level group(P<0.01). The total surgical efficacy was significantly higher in multi-level group than that of UPPP group(84.6%vs52.6%,χ2=13.63,P<0.01).In UPPP group,just AHI,LSa O2 and TS90% were improved compared with those before operation.The difference of AHI,LSa O2,TS90%,the total score and four dimensions scores in SAQLI,ESS score,SS score between multi-level group and UPPP group after operation were significantly(P<0.05).Conclusion:Multi-level surgery is a safe and feasible treatment method for moderate to severe OSAHS. The evaluation by subjective and objective ways can more comprehensive accurately reflect the surgical efficacy and effects of disease on patients, life quality.
