Objectives To discuss vessel wall imaging features of vertebrobasilar artery dissection by using 3T high resolution MRI.Clinical imaging classification of vertebrobasilar artery is proposed on the basis of clinical findings, arterial wall imaging features, in order to make guidenance for treatment strategies. Methods:12 Patients with 12 vertebrobasilar artery dissection are enrolled consecutively from December 2013 to May 2014.3T HR-MRI is performed to make diagnosis of vertebrobasilar artery dissection and to category its angiography patterns into aneurysmal dilation, steno-occlusive, combined or unclassifiable pattern.and the corresponding treatment plan of various types of dissecting is different. Each lesion is scaled based on its wall imaging features, luminal configuration, location, size, perforator involved and clinical symptoms. Treatment methods and the results after six months the review are also recorded. Statistical analysis is performed using SPSS13.0.Results Typical features include vertebrobasilar dissection intimal flap (67%, n= 8), dual-chamber levy (58%, n= 7) and intramural hematoma (33%, n = 4) can be high-resolution magnetic resonance imaging arterial wall imaging and enhanced scanning recognition. According to high-resolution magnetic resonance imaging credits display incomplete intimal dissection 8 cases (image type Ⅰ) undergo stent and coil embolization, two cases review again in coil embolization. Intimal complete four cases (image type Ⅱ, Ⅲ) are dynamic observation,1 case of the image type Ⅱ is self-healing and 3 cases of image type Ⅲ are no significant change six months after the review.Conclusions 3T high-resolution magnetic resonance imaging to clearly show the vertebrobasilar artery dissection of the vessel wall imaging changes, help researchers vertebrobasilar dissection of the vessel wall pathology and intimal lesion integrity. And high-resolution magnetic resonance imaging based on typing significantly raise guidance for further vertebrobasilar dissecting intervention treatment.
