Objective:to explore the DCE MRI method and receiver operating characteristic (ROC) method for identification of head and neck malignancy intumescent lymph node of value. Method:collect between September 2014 and February 2014 in xinjiang medical university first affiliated hospital of 16 patients with cervical lymph node enlargement (a total of 29 swollen lymph nodes), were confirmed by biopsy or surgical pathologic examination, according to the result of pathology were divided into benign group (6 cases), malignant group (10 cases), DCE-MRI examination before operation, respectively, in the parameter on images that draw the interest measure, measure the corresponding Ktrans value, Kep value and value of Ve, and measurement results is analyzed using ROC curve method. Results:benign group Ktrans values average was 0.110+/-0.036 min-1; Malignant group Ktrans the average was 0.463+/-0.121 min 1, benign group the average was 0.214+/-0.086 min 1, malignant group Ve the average was 0.349+/-0.159 min 1, benign group Kep the average was 0.547 +/-0.323 min 1, malignant group Kep values the average was 1.631+/-1.882 min 1, Ktrans value, Ve and Kep the area under the curve were 0.973,0.765,0.772, ROC curve value to the diagnosis of benign and malignant of point were 0.133,0.181,0.7685, and the corresponding sensitivity and speciality rate were 100%,83.3%,88.2% and 50%,88.2% and 83.3%. Conclusion:DCE MRI Ktrans, Ve and Kep parameter for neck swelling differential diagnosis of benign and malignant lymph nodes were statistically significant, ROC curve method can provide three parameters in the diagnosis of benign and malignant lymph nodes, and Ktrans value in the diagnosis of highest efficiency.
