Objective:Our purpose herein is to observe changes in mean platelet volume as an influential indicator in patients with pulmonary artery hypertension associated with secundum atrial septal defect (PAH-ASD) and the influence of PAH-ASD. Methods:We chose 627 patients who suffered from secundum ASD and were diagnosed in the First Affiliated Hospital of Xinjiang Medical University Heart Center between January 2010 and March 2014. Patients were divided into two groups based upon whether they had PAH or not. One group included 420 patients who had PAH and the other included 207 without PAH. In addition, we also chose 690 healthy people as a control group who were diagnosed at the same time as the experimental group. We measured platelet and other blood parameters in all patients. We also performed right heart catheterization in some of the patients who suffered from secundum ASD. Results:(1) The MPV was significantly higher in secundum ASD patients than in control group (P<0.001). No significant difference was observed in MPV between PAH group and non-PAH group (P=0.268). (2) In univariate and multivariate logistic regression analysis, age (OR=1.401,95% CI 1.087 to 1.806, P=0.009), RV diameter(OR=1.101,95%CI1.026 to 1.181, P=0.008) and ASD defect diameter (OR= 1.064,95% CI 1.013 to 1.118,P=0.013) were independent predictors of PAH-ASD. Conclusion:Our study demonstrated that Age, RV diameter and ASD defect diameter were significantly associated with PAH-ASD. Platelet activation exists in PAH-ASD, but this may not the precipitating cause that contribute to the pathogenesis of PAH-ASD. MPV at admission does not predict the risk of PAH in patients with secundum ASD.
