| Objective:The use of qualitative interviews and SWOT through the analysis and research of the problem of Xinjiang chronic diseases health management. To design the benefit maximization of the path of the chronic diseases health management on this basis.Methods:The study on the basis of the literature review, the patients by employing the method of simple random sampling to learn about the patient’s health management present situation, through the depth interview method to master the current medical institutions of chronic disease health management measures, using the SWOT analysis method to determine the community chronic disease health management present situation and the slow disease health management problems that exist in the development process. Results:(1)By follow-up of 462 patients with an implanted pacemaker, mainly middle-aged and old patients. The patient’s health management related knowledge awareness is higher, but poor adherence; The patient’s health management confusion, single demand, lack of the cognition of health management. (2)Xinjiang health agency showed a trend of increasing in number, especially the rising trend of community health service centers is more apparent. (3)The work of the community health service institutions is multifarious,it’s difficult to carry out the follow-up,and only few physicians manage many residents, equipment and insufficient funds. Conclusion:Xinjiang chronic diseases health management need to constantly reform, strengthening human resources training, powerful role play of the government and the medical institution, to establish a full of vitality of the chronic disease health management linkage management mechanism, and to create a good development opportunity for the chronic disease health management in Xinjiang. |
