| Objective: To investigate heart rate variability(HRV) in patients with acute stroke and analyzes the characteristics of HRV in stroke patients.Methods: 72 acute cerebral apoplexy patients were divided into hemorrhagic stroke(37 cases) and ischemic stroke(35 cases) according to the results of CT and MRI, selected 80 healthy subjects as control. Monitor HRV for 24 hours to all test subjects, and detect HRV again for patients after one month treatment. Using multiple linear stepwise regression method carries on the analysis to the HRV in patients.Results:(1) There were no significant difference of NIHSS(America National Institutes of Health Stroke Scale score) score between hemorrhagic stroke and ischemic stroke after 24 hours admission.but there is significant difference after one months of therapy of two groups in the score of NIHSS.(2) Compared with control, the parameters of HRV: SDNN, SDANN, r MSSD, PNN50, TP, LF and HF were significantly decreased, while LF/HF increased in patients.(3) Compared with 24 hours hospitalization, the parameters of HRV: SDNN, SDANN, r MSSD, PNN50 and LF were significantly increased after 1 months treatment, but TP, HF and LF/HF were no significant difference in patients.(4) Compared with control, the parameters of HRV: SDNN, SDANN, r MSSD, PNN50, TP, LF and HF were significantly decreased both daytime and night, while LF/HF increased in patients. The HRV were no significantly difference between daytime and night after 24 hours admission, but after 1 month treatment, there were significantly higher than 24 hours admission, and all the parameters of HRV: SDNN, SDANN, r MSSD, PNN50, TP, LF, HF and LF/HF were significantly higher during night than that of daytime; Except for LF/HF, the SDNN, SDANN, r MSSD, PNN50, TP, LF, HF were significantly increased after 1 month treatment than after 24 h admission in patients.(5) Using age, sex, NIHSS score and mean arterial pressure as independent variables, multiple linear stepwise regression analysis showed that SDNN and HF were significantly negative correlation with NIHSS score, the mean arterial pressure in patients, and LF was significantly negative correlation with NIHSS score.Conclusion:(1) The HRV of day and night of fluctuation rhythm were weakened or disappeared, in stroke patient. With the reprovement of disease, the sympathetic gradually coordination, HRV rhythm gradually restore.(2) HRV in time domain and frequency domain indices were significantly and negatively correlated with NIHSS scores and the average dynamic pressure in stroke patients. |
