Objective:To assess the value of left ventricular remodeling in patients with coronary disease (CAD) by real-time three dimensional echocardiography (RT-3DE).Methods:A group is thirty healthy volunteers.65cases of coronary heart disease patients,according to the results of coronary artery angiography, single main vessel stenosis which was equal to or larger than50%was defined as recruited standard. The lesion group was divided into3groups by results of coronary artery angiography, B group (n=19) was patients with left circumflex coronary artery or/and right coronary artery stenosis; C group (n=24) was patients with left anterior descending coronary artery stenosis; D group (n=22) was patients with double or triplex coronary arteries stenosis which must include left anterior descending coronary artery. The left ventricular end diastolic volume(LVEDV),end systolic volume(LVESV),ejection fraction(LVEF), LV mass(LVM),LV remodeling index(LVRI) were measure by2DE biplane Simpson’s method and RT-3DE.The inter-group differences of LVRI, intra-group differences, correlations of LVRI and correlations between LVRI and LVEDV or LVEF in all groups were analyzed.Results:(1) The RT-3DE and2DE LVEDV, LVESV, LVM significantly increased in order of A group, B group, C group, D group, respectively(P<0.05or P<0.01); but LVEF,LVRI significantly decreased,respectively(P<0.05or P<0.01).(2) LVEDV, LVESV, LVM,LVRI measured by RT-3DE and2DE were significantly different inter-group, in C group and D group(P<0.05),but no significantly different in A group and B group(P>0.05).(3) The correlation analysis indicated that:There were significant correlations between RT-3DE LVRI and2DE LVRI in A group, B group, C group (P<0.01), but for D group (P>0.05). There were significant negative correlations between LVRI and LVEDV, significant positive correlations between LVRI and LVEF in B group, C group, D group,respectively (P<0.01); but without significant correlations(P>0.05) in normal controls group.Conclusions:RT-3DE can quantitatively evaluation of left ventricular remodeling and function in patients with coronary heart disease.RT-3DE LVRI could take as a new approach for evaluating left ventricular remodeling of coronary artery disease in clinic superior to2DE. |
