Objective To investigate the efficacy of flubiprofen axetil combined with fentanyl for Intravenous patient-controlled analgesia (PCIA) after laparoscopic radical resection of cancer.Methods Sixty ASA Ⅰ or Ⅱ patients, aged30-65yr, scheduled for laparoscopic radical resection of cancer under general anesthesia, were randomly divided into A、B、C groups(n=20).PCIA was performed in all the patients at the moment of the end of surgery. In group A, PCIA was performed with fentany15ug/kg in120ml of normal saline. In group B, PCIA was performed with fentany10ug/kg and flubiprofen axetil6mg/kg in120ml of normal saline. In group C, PCIA was performed with fentany lOug/kg and flubiprofen axetil4mg/kg in120ml of normal saline. The PCIA pump was programmed to deliver0.5ml per demand with a15min lockout interval and background infusion at a rate of2ml/h. When VAS score≥4, Tramadol100mg was injected immediately.VAS、BCS、Ramsay scores and BP、HR、RR、SPO2were recorded at lh after extubation and4、12、24、48h postoperatively.The total press times of PCIA、remedial measures and side effects were valuated within48h after surgery. Took venous blood samples for the examination of the serum concentrations of AST、ALI、BUN and Scr before the operation and at48h after surgery.Results BP、HR、RR、SPO2、liver function and renal function had no significant difference between the three groups, and were all in the normal range at each time point.Compared with T1, in group A and B, the VAS and BCS scores were not significantly different. While in group C, the BCS score were decreased at12h after surgery, and increased at48h after surgery, the VAS scores were significantly decreased at24、48h after surgery. The Ramsay scores were significantly changed at12h、24、48h after surgery among three groups Compared wit group A, the VAS、BCS scores had significant difference at4、12、24、48h after surgery in group B; the BCS scores in group C were lower significantly at12h after surgery, and VAS scores was higher significantly at1h、4h、12h.While the Ramsay score was lower at12h in group B and at1、12h in group C. Compared with group B, the BCS in group c scores were lower at T4~T48, the VAS scores were higher at T1~T48.The press times in group A were more than that in group B, and less than that in group C. There were two patiens who needed remedial measures in group A and five patients in group C, while there were no patient who needed remedial measures in group B.The side effects in group A were extremely more than that in group B, C.Conclusion1. PCIA with flubiprofen axetil6mg/kg and fentany lOug/kg provides better efficacy for postoperative Analgesia after laparoscopic radical resection of cancer, and has little effect on functions of liver and kidney.2. PCIA with flubiprofen axetil4mg/kg and fentany lOug/kg is not satisfactory for Analgesia after laparoscopic radical resection of cancer.
