ObjectiveTo investigate the efficacy of patient-controlled intravenous analgesia (PCIA) with different nacortics combined with lappaconitine for postoperative gynecologic analgesia.MethodsEighty patients, aged30-60years old, scheduled for gynecological operation, were randomly divided into four groups (n=20, each):Group A received morphine0.6mg/kg and lappaconitine16mg for PCIA, group B received dezocine0.6mg/kg and lappaconitine16mg for PCIA, group C received butorphanol0.12mg/kg and lappaconitine16mg for PCIA and group D received fentanyl8μg/kg and lappaconitine16mg for PACI. Each groups of PCIA were added ondnsetron hydrochloride5mg. The use of PCIA infusion before, give the load:group A morphine5mg; group B dezocine5mg; group C butorphanol1mg; group D fentanyl0.05mg. The PCIA parameter was set to2ml/h continuous infusion.Receives pressed pump boRom as needed to get abolus does of0.5ml volume eactime,with lockout time of15min.The data of SBP, DBP, HR and SpO2were recorded at1h,6h,12h,24h,36h or48h postoperation, and Visual analogue score (VAS), Bruggrmann comfort scale (BCS), Ramsay scores and the side effect of PONV, dizzy, headache and respiratory depression were also recorded, respectively.Results1. The VAS score:1h after operation, VAS score from high to low was group C, group D, group A, group B. Compared with group C, the score of A,B,D groups were lower, especially group B (p<0.05).6h after operation, VAS score from high to low was group C, group D, group B, group A. Compared with group C, the score of B groups were lower(p<0.05), Other groups showed no statistical significance.2. The BCS score:1h after operation, Compared with group C the score of B groups were higher (p<0.05).3. Adverse reactions:the incidence of nausea, vomiting, A group is obviously higher than other groups (P<0.05) headache and dizziness of A group and C group have appeared, but not statistically significant; the four groups had no skin itching and respiratory depression. 4. Ramsay scores:Each group at different time after the operation the patient vital signs monitoring index and Ramsay scores showed no significant differences (P>0.05).ConclusionCombined with morphine0.6mg/kg, fentanyl8μg/kg and butorphanol0.12mg/kg, dezocine0.6mg/kg combined with lappaconitine16mg for PCIA provide effective analgesia with less adverse reactions for postoperative gynecological analgesia.
