Dy On The Constitution Of Traditional Chinese Medicine About IgA Nephropathy And The Relevance Between Its Constitution And Kidney Injury Molecular-1

Objective:By using the theory of TCM constitution,this study, combined with reflecting markers of renal injury degree---Kim-1, rexplored the predisposition of patients with IgAN in Chinese medicine,and after the onset of the mass change law and prognosis,the relevance between constitution and the severity of the disease and prognosis.An objective basis will be provided for clinical medical workers in early preventing and treating IgAN, so as to early detection, treatment and delaying the disease, improve quality of life of patients with IgAN.Methods:Firstly, according to a unified "Chinese constitution classification and decision table"(see Appendix1) and physical assessment standards (see Appendix2) that were develop-ped by CACM in2009. Each constitution relates to6-8problems, each question contains5levels according to1-5scores(the markedissues divided into reverse score according to5points,4points,3points,2points,1point)., questionnaire will be carried on in patients who met the inclusion criteria.Secondly,collecting urine sample:The study involved the relationship between the Constitution and the severity of the diseas,in order to improve the accuracy of the results, so the urine collection need the initial screening and rescreening.(1)preliminary screening: collected second days morning urine10ml (mid-stream of urine)from patients with glomerular hematuria,proteinuria,respectively to2000rpm,centrifugal for10minutes,taking supernatant, storing in-20℃refrigerator.(2) Rescreening:retain only the urine specimens from The initial diagnosis of IgAN through renal biopsy in patients.Finally,The basic situation were collected, including sex, age, course of disease, medical history, renal biopsy, urine routine,the24-hour urinary protein,BP arid so on. correspondding statistical analysis, rexplored the predisposition of patients with IgAN in Chinese medicine,and after the onset of the mass change law and prognosis, the relevance between constitution and the severity of the disease and prognosis.Results:1、Most of the IgAN patients’main TCM constitutions are Qi-deficiency constitutions (30%), Yin-deficiency constitutions (23.3%), Damp-heat constitutions(20%).2、The age distribution, the Constitution Types in IgA nephropathy,18-30-year-old patients with Damp-heat constitutions(34.5%);31-45-years-old,46-66-year-old patients with Qi-defi ciency, Yin-deficiency constitutions.3、IgA nephropathy in different clinical type, Qi-deficiency,Yin-deficiency,Damp-heat, Blood-stasis, normal constitution were dominated by hematuria as ymptomatic abnormal urine and solitary microscope.4、IgA nephropathy Lee’s classification mainly grade Ⅱ, Ⅲ, Ⅳ, Qi-deficiency and Yin-deficiency constitutions in grade Ⅱ,Ⅲ for the main, damp-heat with grade IV mainly.5、From the course of the disease, IgA nephropathy patients in the short course (less than6months),TCM constitution with dampness-heat constitution,accounted for37.5%,but time going on,gradually decreasing; at the same time,Di-deficiency ratio increased gradually, the course of the disease was more than3years, the proportion of patients with Qi-deficiency significantly increased, the largest proportion reached44.8%.6、In IgA nephropathy3main type of constitution, Yin-deficiency, Qi-deficiency constitution compared with patients with damp-heat constitution, the patients with Kim-1in urine were significantly increased (P=0.000<0.01); compared with Damp-heat, patients with Qi-deficiency,Yin-deficiency constitution Kim-1in urine were significantly lower (P=0.000<0.01), but the comparison there was no significant difference the patients with Qi-deficiency and Yin-deficiency of Kim-1in urine (P=0.405>0.05).Conclusion:1、Most of the IgAN patients’main TCM constitutions are Qi-deficiency constitutions,Yin-deficiency constitutions and damp-heat constitutions.2、The constitution of IgAN in young patients with dampness-heat is more common, in elderly patients with Qi-deficiency based virtual Constitution.3、IgAN in different clinical type, Qi-deficiency, Yin-deficiency,Damp-heat, Blood stasis, normal constitution were asymptomatic Hematuria urinary abnormalities and solitary microscopic.4、IgAN Lee’s classification mainly Ⅱ, Ⅲ, Ⅳ, Qi-deficiency and yin-deficiency in Ⅱ, Ⅲ, for the main, damp-heat with grade IV mainly.5、IgA nephropathy patients in the course of the disease is short, with Dampness-heat; the longer course, with Qi-deficiency.6、Patients with IgAN in common constitution, acute onset, damp-heat constitution of patients with urinary Kim-1increased obviously, the condition of the disease is serious; Qi deficiency and Yin deficiency increased between urine Kim-1in patients with no statistical difference and the elevated is relatively low, thecondition is relatively light...
