Objective:To research on the mechanism of Qingrun Yangmu oral liquid in the treatment of dry eye rabbit model and observe the improvement of the quality of life of the syndrome of liver and kidney yin deficiency of dry eye patients by Qingrun Yangmu recipe (free Decoction), and comprehensive evaluation the intervention effect of Qingrun Yangmu oral liquid for the syndrome of liver and kidney yin deficiency of dry eye.Methods:1. Experiment study The Big-eared Japanese rabbits was randomly divided into4groups:normal group, model group,high dose group and low dose group. The normal group without any treatment but adaptive breeding, the other three groups were making the model of TCM syndrome type and eye dry. FL, BUT and sit were examinated by the time point of befor modeling, immediately after modeling,2weeks after modeling.and4weeks after modeling.After given Qingrun Yangmu oral liquid and normal saline for28days, killed the rabbits and taken out the eyes. The effect of Qingrun Yangmu oral liquid on the pathological changes of experimental rabbit model’s cornea and the expression of inflammatory cells, goblet cells and Mucin(MUC5AC) were observed.2. Clinical observation36patients were collected from outpatient with the diagnosis of dry eye. They were divided into test group and control group randomly. All patients filled in the symptom score and suspended all treated od dry eye. The test group was treated with Qingrun Yangmu recipe (free Decoction) and Sodium Hyaluronate Eye Drops, while the control group treated with Sodium Hyaluronate Eye Drops. Both groups were underwent14days of continuous medication. Filled in the symptom score again. Observing the improvement of the quality of life of the dry eye patients by the score result.Results:1. Experiment study (1) CFS、BUT、sIt:2weeks after-modeling compared with immediately after modeling, the CFS of model group,high dose group and low dose group were reduced, the BUT and sIt were prolonged. Both the high dose group and low dose group were highly significant different compared with the model group.(P<0.05).4weeks after-modeling, the three indicatorsthe of high dose group and low dose group were highly significant different compared with the immediately after modeling and2weeks after-modeling(P<0.05),.but the model group had no significant change compared with2weeks after-modeling(P>0.05).(2) Pathological observation:①HE of Cornea:The control group, each layer of the cornea-s organizational structure was complete, the corneal epithelial had smooth and continuous surface, flat epithelial cells arranged in neat rows, no cell depigmentation, erosion, etc. The model group, the corneal epithelial was not very continuous,and many epithelial cells, erosion, even full-thickness depigmentation were visible. The microscope changes of high dose group had no significant difference compared with low dose group,but these changes reduce the different degrees compared with the model group.②HE of conjunctiva:The control group, conjunctival epithelium and lamina propria were no significant inflammatory cell infiltration,but it had different levels of expression in the other three groups, the expression of inflammatory cell of fornix was stronger than the bulbar conjunctiva department,the model group was stronger than the high dose group and the low dose group(P<0.05).③goblet cells and Mucin(MUC5AC):the expression of goblet cells and Mucin(MUC5AC) of the model group was obvious stronger than the high dose group and the low dose group(P<0.05). Except for the control group, there was no significant difference in the other three groups (P>0.05)2. Clinical observation Before the treatment, there was no significant difference between the groups in age, course, various indicators of symptoms and signs CP>0.05).After treatment, the total effective improvement rate of Western medicine symptoms of test group is94.12%, and that of control group is70.59%, the effect of test group was better than control group(P<0.01); The total effective improvement rate of TCM symptoms scores of test group is94.12%, and that of control group is41.18%, the effect of test group was better than control group (P<0.05).Conclusion (1) The method of modeling which enraging and suturing their eyelids to the skin combined the partial removal of the third eyelid was successful.(2) The efficacy of treatment of dry eye rabbit model by Qingrun Yangmu oral liquid:① Increase the tear secretion;②Maintain the stability of the tear film;③Reduce inflammation of conjunctiva;④Promote the secretion of goblet cells and Mucin(MUC5AC)⑤Effectively alleviate the syndrome of yin deficiency of liver and kidney.(3) Qingrun Yangmu recipe (free Decoction) can improve the quality of life in eye dry patients of syndrome of yin deficiency of liver and kidney,and if combined with Sodium Hyaluronate Eye Drops,the effct would be better.
