Objective①By measuring the hospitalized elderly patients the skin moisture content, oil content, PH value of the hospitalized elderly patients to study the differences of them, strengthen the community management of pressure ulcer and provide baseline data rating.②Using Logistic regression analysis to study the risk factors of inpatients Braden classification according to the Braden rating and elderly patients skin barrier and other factors. In order to improve the predictive validity of the Braden grades and provide basis for clinical prevention and treatment of elderly patients with pressure ulcer, so as to reduce the incidence of pressure ulcers, to ensure the safety of patients and improve the quality of basic nursing.Methods①To investigate risk factors of patients with pressure ulcers262cases of elderly patients were investigated by self-designed skin barrier assessment and the elderly patients pressure ulcer risk watch list. The former includes the patient’s general situation, the Braden scale and skin test results. The latter includes age, BMI, blood pressure, disturbance of consciousness, hemiplegia, paraplegia, serum albumin, parenteral nutrition, bed time, urinary incontinence, urethral catheterization, diarrhea, skin moisture, oil, PH value, a total of15factors.②Detection skin barrier factors Patients were told that the instrument used for testing were nontraumatic before being observed and were informed the research purpose, research process in order to obtain the subjects and their relatives’consent. Testing parts include the tail lumbosacral region, shoulder, the femoral greater trochanter and heel, which a total of four pressure sores often occurred in the area.③Statistical methods to data processing using SPSS20.0statistical software, Measurement data expressed as a percentage, using the chi-square test; Measurement data to x±S, using t test; Using correlation analysis, the rank and inspection. The statistically significant variables as a multiple Logistic regression analysis of the independent variable, Braden classification respectively as the dependent variable of Logistic regression variance, regression analysis. Difference was statistically significant (P<0.05).Results①262new hospital patients were collected in a third rate of Hangzhou hospital cadre ward’s between July2013and June2014, Braden risk score for mild118cases,68cases of moderate risk,46cases of highly dangerous,30cases of extremely dangerous.②Different sex, disease diagnosis of patients with skin test result is not significant, there are differences between the test results with different parts, the heel skin moisture content was significantly lower than that of other parts.③The single factor analysis showed that age, blood pressure, hemiplegia, paraplegia, serum albumin, bed time, urinary incontinence, diarrhea, skin moisture, skin PH patients Braden classification is different, the difference was statistically significant (P<0.05or P<0.01).④Analyzing the10factors results of single factor analysis of statistical sense by multivariate Logistic regression, the results showed that the age, the skin PH value, blood pressure, hemiplegia, bed time five and Braden grading highly correlated (P<0.05).Conclusion①Age, the skin PH value, blood pressure, hemiplegia, bed time is independent risk factors for elderly patients with pressure sore Braden classification. Age is independent risk factors for the development of pressure ulcers occur, with the growth of the age, the risk of pressure ulcers is higher. The higher the skin PH value, the higher the risk of pressure ulcers. High blood pressure are protective factors of pressure ulcers (B value are negative). But low blood pressure significantly increase the risk of pressure sores are not seen, which fact that may be related to the lowering blood pressure patients are most diastolic blood pressure low in this study, basic diastolic blood pressure is between55to60mmHg. Extremely high risk patients with hemiplegia occurred the risk of pressure ulcers is51.137times that of the patients with mild risk. Staying in bed for more than1months than less than1months is of high risk for pressure ulcers.②The patient’s own skin barrier and other factors existing in pressure ulcer risk classification, therefore skin of pressure ulcer risk and other factors should be put into consideration when evaluating the risk classification of risk factors for pressure ulcers.③Many risk factors influence elderly hospitalized patients pressure ulcer development, we should use the principle of evidence-based medicine by reference to domestic and foreign determined risk factors and prevent the occurrence of pressure sores in hospitalized elderly patients by target.
