To observe the efficacy and safety of combined treatment with angiotensin II receptorblocker (ARB) plus angiotensin converting enzyme inhibitor(ACEI) on early diabeticnephropathy.Methods:Randomized controlled trials (RCTs) were searched from CoChrane library, PubMed, Ovid,CNKI, CBM and VIP. Eighteen RCTs which involved1499patients were included and wereanalyzed by Rev Man5.1software.Results:Compared with ACEI mono, combined treatment with ACEI plus ARB was more effectiveon urinary albumin excretion, systolic pressure, diastolic pressure. The BUN of thosereceiving ACEI combining with ARB treatment were slighty lower than ACEI alone, andthere are no significant differences on Serum potassium and serum creatinine. Compared withARB mono, combined treatment with ARB plus ACEI was more effective on Serumcreatinine, but there are no significant differences on systolic pressure, diastolicpressure,serum potassium and serum creatinine. The BUN of those receiving ACEIcombining with ARB treatment were slighty lower than ARB alone.Conclusion:According to the Meta analysis, combined treatment with ARB plus ACEI is safer anduseful on early diabetic nephropathy. However, for the insurmountable limitations of thisMeta analysis, the conclusion needs to be tortured by many other clinical trials of higherquality.
