Objective: Observe the clinical curative effect of Lumbar Disc Herniation bythe treatment of Percutaneous Endoscopic Laser Discectomy and conservativetreatment.Methods: Retrospective analysis from January2013to December2013inshandong university hospital of traditional Chinese medicine in the diagnosis oflumbar disc herniation treated were47patients clinical and imaging data, diagnosticcriteria and clinical research into the standard and follow-up of patients in time.According to different treatment methods are divided into Percutaneous EndoscopicLaser Discectomy in treatment group and the conservative treatment group, thetechnology of Percutaneous Endoscopic Laser Discectomy treatment group of25cases,22cases of conservative treatment group. Record all cases of the two groupsbefore and after treatment at the end of the72h,1month after treatment, follow-uptime (march to June longest shortest) VAS score, low back pain JOA score, ODI index;All the data with mean standard deviation(X±S) said. Groups within the mean(X±S) paired t test, between groups using mean (X±S)independent samplet-test;: difference was statistically significant (P <0.05, P <0.01, there are significantdifference for the statistical significance. Draw the two groups before treatment,treatment after72h,1month after treatment, the VAS score of the last follow-up,JOA score and ODI disability index continuity average contrast table of two groups oflow back pain JOA score at the time of the last follow-up period. And uses theimproved MacNab therapeutic effect evaluation at the end of the two groups after treatment of standard time follow-up results.Results: The group comparison: conservative treatment group before and aftertreatment VAS score were significant statistical significance (P <0.01, table1).Percutaneous Endoscopic Laser Discectomy treatment group before and aftertreatment VAS score were significant statistical significance (P <0.01, table2);Conservative treatment group before and after treatment JOA scores were significantstatistical significance (P <0.01, table3). Percutaneous Endoscopic LaserDiscectomy treatment group before and after treatment JOA scores were significantstatistical significance (P <0.01, table4). Conservative treatment group before andafter treatment ODI index had significant statistical significance (P <0.01, table5).Percutaneous Endoscopic Laser Discectomy treatment group before and aftertreatment ODI index had significant statistical significance (P <0.01, table6).Comparison between groups: conservative treatment group and PercutaneousEndoscopic Laser Discectomy VAS score before treatment group has no statisticalsignificance (P>0.05, table7),72h after the treatment,1month after treatment, VASscores at the time of the last follow-up have significant statistical significance (P <0.01, table7). Conservative treatment group with Percutaneous Endoscopic LaserDiscectomy JOA score before treatment group has no statistical significance (P>0.05,table8),72h after the treatment, treatment after1month when the JOA scores weresignificant statistical significance (P <0.01, table8), at the time of the last follow-upJOA score was statistically significant (P <0.05, table8); Conservative treatmentgroup and Percutaneous Endoscopic Laser Discectomy treatment group before andafter treatment,72h ODI index has no statistical significance (P>0.05, table9),1month after treatment, at the time of the last follow-up ODI index was statisticallysignificant (P <0.05,9); Conservative treatment group, treatment group patients withPercutaneous Endoscopic Laser Discectomy JOA score the last follow-up period: theconservative treatment group1patients RIS>75%, and50%in4patients <RISacuities were75%,24%in11patients <RIS acuities were50%,0<6patients RIS24%or less. Percutaneous Endoscopic Laser Discectomy treatment group,13patientswith RIS>75%,8patients were50%<RIS acuities were75%,24%of3patients < RIS acuities were50%,0<1patients RIS24%or less. At the end of the two groupsafter treatment time modified month follow-up MacNab curative effect evaluationstandard, which the optimal4cases of conservative treatment group, benign and11cases, can be in5cases and worse in2cases, fine rate was65.21%; PercutaneousEndoscopic Laser Discectomy treatment group16cases,6cases,3cases,0cases, finerate was84.00%.Conclusion: Whether conservative or PELD treatment of LDH short-term hascurative effect; PELD short-term curative effect of treating LDH better thanconservative treatment group. PELD has simple operation, small trauma, less pain,rapid recovery, fewer complications, and impact on the quality of life after smaller,and many other advantages, But the long-term curative effect remains to be seen.
