| ObjectiveTo compare the clinical effect of operative treatment and nonoperativetreatment for elderly patients with displaced intra-articular calcaneal fractures.Clinical Material and MethodsSixty-two patients who presented to the Jinan Military General Hospitalfrom2001JAN to2012DEC with an intra-articular calcaneal fracture with2mm of displacement were evaluated between operative teratment groop with32patients and nonoperative treatment groop with30patients. Independentobservers followed the two groups radiographically and clinically at least twoyears. Bohler angle and Gissane angle were measured preoperatively andpostoperatively.Meanwhile,we evaluated foot function which was assessedwith Maryland foot score system and incidence rate of complication.ResultsThe two groups were comparable with respect to age, sex, and fracturetypes. All cases were followed up at least two years. Nonoperative patients werefollowed up for24to37months,mean27.8±6.6months. Operative patients werefollowed up for28to39months, mean29.8±5.8months. All fracture healedwhen we finished the follow-up. Bohler angle and Gissane angle which weremeasured preoperatively and postoperatively had significent differenceP<0.01.Calcaneal width which were measured preoperatively and postoperatively hadsignificent differenceP<0.01. When assessed foot function with Maryland footscore system, nonoperative groop scored56to85,mean67.02±12.3. Andoperative groop scored75to95, mean81.1±12.8. Foot function of two groopshad significent difference. ConclusionsGood clinical result could be obtained with operative treatment in elderlypatients with displaced intra-articular fractures of the calcaneus. Open reductionappears to be an acceptable method of treatment for displaced calcanealfractures in elderly patients if they have good general conditions．... |
