Abstract/Summary:
Objective Through monitoring the level of the peak and platform pressure of airway,airwayresistance, the total dynamic lung compliance, arterial blood gas and coagulation function inmechanically ventilated patients with AECOPD to compare the effects of different oxygen flow ratedriven inhalation of heparin on respiratory mechanics in mechanically ventilated AECOPD patients. Toobserve the improve of respiratory mechanics, finding suitable oxygen flow, Provide a theoretical basisforAECOPD patients with mechanical ventilation select appropriate oxygen flow.Method51patients with AECOPD mechanical ventilation were randomly divided into A、B、C threegroups. Group A (17cases) were given the oxygen flow rate of5L/min to atomizing inhalation groupB(19cases) were given the oxygen flow rate of7L/min, group C (15cases) were given the oxygen flowrate of9L/min. Respiratory mechanics、arterial blood gas and coagulation function were detected beforeand after automizated inhalation in30min,1,3,7days in all patients. Repeated measurement data usingSPSS17.0statistical software for statistical analysis, all measurement data are presented as mean±standard deviation (x S) under different oxygen flow compared using repeated measuresANOVA.Results After aerosol inhalation of heparin for AECOPD patients with mechanical ventilation, therewere statistically significant to mechanics under for three groups of patients with respiratory level ofdifferent oxygen flow rate (P<0.05),Compared with group A and C, Patients in group B hadobviously improved to the peak and platform pressure of airway, airway resistance, the totaldynamic lung compliance and were statistically significant (P<0.05); The group A and group C had nostatistical significant (P>0.05). In the same group of different monitoring point time, Compared withbefore atomization, in group B, Patients had significantly improved to the peak of and platformpressure of airway, airway resistance, the total dynamic lung compliance at each time point (P<0.05);Patients in group A had changed to each respiratory mechanics after atomization30min(P<0.05); but didnot change obviously in group C (P>0.05). Compared with there groups, Prothrombin time(PT), activatedpartial thromboplastin time(APTT)and fibrinogen(FIB) were no significant difference (P>0.05).Conclusion The best oxygen flow rate was7L/min for heparin inhalation in mechanically ventilatedpatients with AECOPD, it can reduce airway pressure and airway resistance and increase chest-lungcomplicance.7L/min oxygen flow heparin atomization inhalation in the treatment of AECOPD patientswith mechanical ventilation, beneficial to the improvement of the effect of the implementation ofmechanical ventilation and ventilation, and the method is simple and easy. Atomizated inhalation heparinfor AECOPD patients by mechanical ventilation is safe and effective, Three groups of patients at the endof the atomizer, did not see clear adverse effects associated with heparin, worthy of clinical popularizationand application.
