Objective: In order to find a new method for differentiation between tuberculouspleural effusion (TPE) and malignant pleural effusion (MPE), we analyzed thecharacteristics of interleukin-22(IL-22) generated by the pleural fluid mononuclearcells (PFMCs) among TPE and MPE, which were stimulated by specific Mtb antigenpeptides including early secreted antigenic target-6(ESAT-6) and culture filtrateprotein-10(CFP-10). Methods:52cases with TPE and35cases with MPE were electedfor this study. Both IL-22and interferon-γ(IFN-γ)kits by e-Bioscience and flowcytometry (FCM) with Bead array were used to measure the IL-22and IFN-γ in PFMCsof their pleural effusion. These PFMCs were cultured and stimulated by Mtb-specificantigen peptides meanwhile, then the concentrations were measured and analyzedstatistically. Results:It was showed that among TPE, IL-22rose significantly in PFMCsof culture fluid supernatant through stimulation of the peptides(P<0.05),and washigher than MPE(P<0.05)significantly. Both concentrations of IL-22and IFN-γ inPFMC of culture fluid supernatant stimulated by peptides were significantly correlatedwithin TPE group(r=0.3485,P=0.0320).ROC curve showed that if Mtb-specific IL-22is used to diagnose TPE, its sensitivity, specificity and accuracy will be90.38%,80.56%and85.23%separately. Its diagnostic efficiency was equal to IFN-γ(P>0.05).Conclusion: Mtb-specific IL-22should become a new biomarker of differentialdiagnosis for pleural effusions. |
