The Application And Research Of Cervical Ripening Balloon,Dinoprostone Suppository, Misoprostol And Low-position Small Water Bag In Induction Therapy

Object:To investigate the application of Cervical Ripening Balloon, Dinoprostone Suppository, Misoprostol and low-position Small Water Bag in induction therapy.Method:From April2014to August2014,168patients, who took induction therapy in Tengzhou Central People’s Hospital, were randomly divided into four groups.38patients in Group A were treated with Cook Cervical Ripening Balloon,60patients in Group B were treated with Dinoprostone Suppository,40patients in Group C were treated with Misoprostol, and30patients in Group D were treated with low-position Small Water Bag. Several parameters in these4groups were compared after the treatment, including the cervical score, duration of regular contractions start, duration of labor, the incidence of early rupture of membranes, the incidence of frequent uterine contractions, the incidence of fetal distress of new birth child, whether it requires use of oxytocin prime or choose other induction methods, the final choice of mode of delivery, the incidence of nosocomial infections, the incidence of birth canal laceration, postpartum newborn score, and postpartum neonatal jaundice. A retrospective study was made in order to explore the differences of effect in these four induction therapies.Result:In induction therapy,the affect of Cervical Ripening Balloon, Dinoprostone Suppository, Misoprostol and low-position Small Water Bag on cervical scores in4groups showed difference which is statistically significant (P<0.05). The effectiveness of the treatments can be sorted as, Dinoprostone Suppository, Cervical Ripening Balloon, Misoprostol, and low-position Small Water Bag. Thought multiple comparison,we found that Dinoprostone Suppository and Misoprostol can start effect Palace easier than Cervical Ripening Balloon and Small Water Bag(P<0.0071, α’=0.0071),multiple comparisonsmultiple comparisonsThe difference of frequent uterine contractionsincidenceis statistically significant (P<0.0071, a’=0.0071). The chance can be sorted as Dinoprostone Suppository, Cervical Ripening Balloon, Misoprostol, and low-position Small Water Bag. The difference of incidence of amniotic is not statistically significant (P>0.05). The difference of necessary of oxytocin prime is statistically significant (P<0.0071, a’=0.0071). The difference of chance of cesarean due to failure of induction is statistically significant (P<0.0071, a’=0.0071). The difference of incidence of birth canal laceration is statistically significant (P<0.05). The chance can be sorted as Dinoprostone Suppository, Cervical Ripening Balloon, Misoprostol, and low-position Small Water Bag. The difference of postpartum newborn score、icterus neonatorum、fetal distress、 hospital infection are not statistically significant (P>0.05).Conclusion:In induction operation, these four treatments, Cervical Ripening Balloon, Dinoprostone Suppository, low-position Small Water Bag, andMisoprostol, are pros and cons in different aspects. Dinoprostone Suppository is quick and efficient, while also with high risk of frequent uterine contractions, high risk of birth canal laceration, and expensive. Low-position Small Water Bag is cheap, with moderated effect, while also with high risk of induction failure due to late uterine contraction, postpartum hemorrhage after cesarean section, the higher the chance of postpartum infection. Cervical Ripening Balloon performs outstanding and safe in Cervical Ripening, while the price is high, and the duration of labor is long due to the need of Artificial rupture of membranes and Intravenous oxytocin after cervical ripening.The price of Misoprostol is low, and with repeated small dose application,no frequent contraction has been reported.The incidence of birth canal laceration has no statistical difference from other treatments. However, it needs repeated application, and repeated vaginal operation. The duration is long, and the incidence of puerperal infection is high if induction is failed.
