Background Atopic dermatitis (AD) is a common, chronic or chronicallyrelapsing skin disease that mainly occurs in children, rash polymorphism andhas a tendency to seep, protracted course of this disease. The cause andpathogenesis of AD complex. Related to the interaction between environmentand genes, also related to immune system. The pathogenesis of AD is also notfully clear, especially how the dysfunctional reactive T cells activate andmaintain active state are still not really clear, which make considerabledifficulties to diagnosis, treatment and prognosis judgment of AD forclinicians. The initial CD4+T lymphocyte subsets accepted antigen stimulationsignal, It has been proved that there are at least four different kinds of CD4+Tcells subsets-Th1、Th2、Th17and Treg cells so far, perform differentbiological functions. These subgroups effect and limited each other andcondition each other through the secretion of cytokines, form a CD4+T cellimmune regulatory network, and plays important roles in the process ofinflammatory diseases and autoimmune disease. Objective To research thechange regular of Th1/Th2/Th17/Treg balance and relevant cytokines in atopicdermatitis(AD)’s peripheral venous blood,and Compare the relationship withthe severity of disease further, investigate the immune mechanism of atopicdermatitis disease, providing the compelling truth to diagnosis and treatmentthe disease, Improving Indisposition suffers, and improving the quality of life.Materials and methods5ml venous blood serum were collected from47patients which were diagnosed with AD, and40normal individuals after allimprove the clinical data of the patient group registration, In accordance withthe severity in patients with AD (SCORAD scores) rating requirements. Under the condition of aseptic, we detected the content of IFN-gamma, IL-4,IL-10, IL-17, IL-21,23and TGF-beta in peripheral blood of patients withAD by Luminex technology and Statistical analysis was carried on, at thesame time, compared with the severity of the lesion (SCORAD scores),Todraw a conclusion that the change regular of Th1/Th2/Th17/Treg balance inAtopic dermatitis.Results:1Statistics suggest that in peripheral blood serum of the patients withAD, IL-4(P<0.05)、IL-10(P<0.05) levels were increased and IL-17(P<0.01)、IL-23(P<0.01)levels were increased significantly than that of control group,However, Although IL-21、IFN–gamma and TGF-beta level weremodification than that of control group, but do not have significant (P>0.05);2The relationship among the Th1, Th2, Th17and Treg cell subsets:IFN-γ/IL-4(P<0.01)、IL-17A/TNF-βincreased significantly(P<0.01)and IFN-γ/IL-17(P<0.01) decreased significantly;3The relationship between the Th1、Th2、Th17、Treg cells relevantcytokines and SCORAD scores:IL-17was positively significant correlationwith SCORAD scores (P <0.01),but IFN-gamma, IL-4, IL-10,IL-21,IL-23and TGF-beta ware little correlation (P>0.05).Conclusions:1Used Luminex technology to detect and obtain a result that IL-4、IL-10were increased and IL-17、IL-23were increased significantly in AD,However, Although IL-21、IFN–gamma and TGF-beta level weremodification than that of control group, but do not have significant.2Confirmed the IL-17was significant positive correlation with theseverity of the lesion, however, IFN-gamma, IL-4, IL-10, IL-21, IL-23and TGF-beta has no obvious correlation with the severity of the lesion. 3Explore the change regular of Th1/Th2/Th17/Treg balance in AtopicDermatitis and their relevant cytokines, come to a conclusion that the changeof Th1/Th2/Th17/Treg balance is the significant pathogenesis of AD.4In the clinical, On the one hand, IL-17can be expected to become aimportant examination index in the diagnosis of AD; On the other hand, wenot only should focus on the treatment of targeted therapy of IL-17, but alsopay attention to the balance of Th1、Th2、Th17、Treg cells.
