| Objective: Colorectal carcinoma is a common malignancy, seriouslyaffecting the health of citizen. The transcatheter is the most common organ ofcolorectal cancer metastasis, and reported nearly23%~51%of patients areassociated with hepatic metastases of colorectal cancer. Surgical resection ofhepatic metastases is the only radical treatment, while80%to90%of patientsdue to various reasons can not be treated with surgery, interventional treatmenthepatic with the transcatheter arterial chemoembolization and transcatheterarterial infusion chemotherapy improve local plasma concentration, whileblocking tumor blood supply, to palliative treatment of hepatic tumors.Methods: collected from June2012to January2014in Bethune FirstHospital of Jilin University admitted to have histological or cytologicalexamination confirmed colorectal carcinoma, two radiographic examinationconfirmed hepatic metastases, was unable to surgical resection, withoutchemotherapy in the same period and according to Karnofsky (KPS) scoreassessment KPS score≥50points, expected survival of patients≥6months,heart and lung function was normal, liver and kidney function in less than threetimes the normal WBC≥3.5x10^9/L, platelets≥50x10^9/L; qualifyingenrolled21patients,15males and6females, mean age53.8years old. Transcatheter arterial chemoembolization combined with transcatheter arterialinfusion chemotherapy total131times, observe the clinical efficacy and qualityof life changes and to estimate survival.Result:1. The primary lesion patients for colonic carcinoma13cases,8cases of rectal carcinoma. Only16cases of hepatic metastases occurred in2patients with abdominal lymph node metastasis,2cases with pulmonarymetastasis,1case with brain metastases. DSA angiography is divided into13cases of the rich blood supply of vascular staining of the tumor, the lack ofblood supply8cases. The group underwent TACE+TAI total131times, theaverage course of treatment is6.2times.2. Evaluation objective response: According to Response EvaluationCriteria in Solid Tumors (mRECIST2010), efficiency is defined as theproportion of complete remission and partial remission of the total sample. Thisgroup of patients achieved complete remission in2cases, partial remission in11cases, the total efficiency is61.90%.3. To the end of the follow-up, this group of patients with a median survivalof14.1months, six-month survival rate is100%,12month survival rate is90.48%.4. Quality of life: According to the Karnofsky (KPS) score assessments toincrease after treatment than before treatment>10points is improved, reducing>10points is dropped, change≤10points is stable, improving the quality of lifeof patients have efficiency of76.19%. Conclusions: Raltitrexed in hepatic metastasis of colorectal carcinoma byTACE combined with TAI have approved clinical efficacy, and prolong survivaland improve quality of life for patients; transcatheter arterial chemoembolizationand transcatheter arterial infusion chemotherapy is a preferential choice forpatients who are unable to surgical treatment with hepatic metastases treatment,tumor blood vessel conditions and whether iodized oil can fully deposition is ansignificant factor affecting the interventional therapy of hepatic metastases. |
