| Background:With China’s rapid economic and social development, people’sliving conditions have been greatly improved, but on the other hand, thepace of life is also getting faster, which leads to serious work stress andwith it comes negative emotions like anxiety or impatience. Thesenegative emotions have become an important factor for the risingincidence of thyroid disease recently, especially the incidence of subacutethyroiditis---seriously affecting a patient’s quality of life. In moresevere cases, some patients will develop to hypothyroidism. Thus theviable treatment options are particularly important. At present, what’sacknowledged by experts both inside and outside of China is that theetiology of subacute thyroiditis is autoimmune disorder caused by viralinfections and the main therapies are nonsteriodal antiinflammatory drugsand adrenocortical hormone. In clinical practice, attention is centered onthe rational using hormones, which means to get the maximumtherapeutic effect by using the minimum hormones. In recent years,intrathyroid local injecton of betamethasone is a new method for thetreatment of subacute thyroiditis.Objective:The thesis aims to explore the clinical effect and prognosis in thetreatment of subacute thyroiditis with different methods. Methods:A retrospective analysis of101cases of subacute thyroiditis from the second hospital of Jilin University, Department of Endocrinology between September2012and September2014was undertaken.The medical records and follow-up data were analyzed. The subjects were randomly assigned into three groups: Group A with oral nonsteriodal anti-inflammatory drug---aspirin, Group B with oral glucocorticoid---prednisone and Group C with local injecton of betamethasone. The author observed the differences among three groups in fever clearance time, regression time of tenderness and thyroid mass,recovery time of ESR, FT3, FT4, TSH as well as differences in side effects and prognosis. The data was analyzed by SPSS21.0statistical software and P-value was defined as less than0.05. Measurement data was detected by analysis of mean±standard variation. Themean values between the two groups were compared using T-test.X2test was adopted in gender comparison. This thesis compared the good or bad therapeutic effect of different approaches.Results:Of the24cases analyzed in Group A with oral nonsteriodalanti-inflammatory drug---aspirin, two showed discomfort of the stomach,and two were identified as treatment failure because their conditionsrepeated. Of the46cases in Group B with oral prednisone, the symptoms of neck pain and goiter disappeared after10to25days. Among them,three appeared hormonal side effects, one stopped taking the medicine onhis own, and seven had relapses.31cases in Group C were treated withlocal injecton of betamethasone. The dosage was5mg each time and itwas given in once every3to7days. After7to15days, the symptoms ofneck pain and goiter disappeared with no obvious influence on bloodsugar and blood pressure. No any other side effects occurred as well.Conclusion:For the treatment of subacute thyroiditis with severe illness statusand long course, intrathyroid local injecton of betamethasone is a kind ofsafe and effective method. |
