| Objectives:To assess the effectiveness of psychology nursing on patients with anxiety duringPCI perioperative period, providing effective evidence for evidence-based practice.To construct a psychological intervention mode on patients with anxiety during PCIperioperative period based on the current researches, providing optimizedpsychological nursing process for clinical practice。Methods:Fllowed by Cochrane Handbook for Systematic Reviews of Interventions.Published researches about psychology nursing on patients with anxiety during PCIperioperative period were searched through CBM,CNKI, VIP, Wan Fang,PubMed,Cochrane library,PubMed, Academic Search Complete (ASC) of EBSCO databaseincluding randomized controlled trial or quasi-randomized control trial from January1,2009to December31,2013. According to the standard, Screening researches,assessing the quality of the included reaearches and extracting the information ofthem. Meta-Analysis was performed by RevMan5.2software.Results:1. Finally,27researches and2457patients were analysed in thise study.28researcheswere divided into3group, the cognitive-behavioral therapy including4articles,themusic therapy group including6articles and the comprehensive psychologicalnursing group including17articles.2. the effectiveness of cognitive-behavioral therapy:Meta-Analysis of SAS differencevalue before operation is [WMD=9.56，95%CI [3.99,15.14]，Z=3.36(P=0.0008)],one day after operation is [WMD=6.15，95%CI [4.02,8.28]，Z=5.65(P <0.00001)],three days after operation is [WMD=9.25，95%CI [6.90,11.60]，Z=7.72(P <0.00001)] and five days after operation is [WMD=7.71，95%CI [5.44,9.98]，Z=6.65(P <0.00001)]。3. the effectiveness of music therapy: Meta-Analysis of STAI difference value is [WMD=8.29，95%CI [3.65,12.93]，Z=3.50(P=0.0005)]. Meta-Analysis of SASdifference value is [WMD=8.79，95%CI [6.42,11.15]，Z=7.27(P <0.00001)].Meta-Analysis of Heart Rate difference value is [WMD=8.60，95%CI [3.59,13.61]，Z=3.36(P=0.0008)]. Meta-Analysis of Systolic Blood Pressure difference value isdifference value is [WMD=16.51，95%CI [6.41,26.60]，Z=3.21(P=0.001)].4. the effectiveness of comprehensive psychological nursing: Meta-Analysis of SASdifference value before operation is [WMD=7.58，95%CI [4.59,10.56]，Z=4.97(P<0.00001)]. Meta-Analysis of SAS difference value on the day of operation is[WMD=6.44，95%CI [4.60,8.27]，Z=6.87(P <0.00001)]. Meta-Analysis of SASdifference value after operation is [WMD=10.36，95%CI [9.52,11.20]，Z=24.09(P <0.00001)]. Meta-Analysis of SAS difference value after nursing is[WMD=8.35[6.69,10.01]，Z=9.88(P <0.00001)]. Meta-Analysis of HAMA difference valueafter operation [WMD=4.68，95%CI [2.43,6.94]，Z=4.06(P <0.0001)].Conclusions:1. The importance of psychological nursing has has been paid attracted extensiveattention in domestic. The performance of nowdays the relative researches’characteris:There are so many types of psychological nursing plan and the plans overlap;Thequality of the included reaearches are moderate; large sample and strongrepresentative researches still lack.2. Psychological nursing can decrease the anxiety level of patients with PCI operation inhospital. The anxiety level of patients can reduced after one day, after three days andfive days of the surgery,with the intervention of Cognitive behavioral therapy,but itsintervention before PCI needs further study; Music therapy, can reduce the anxietylevels of patients after PCI, but its intervention on physiological index and theevaluation results of STAI scale, still need validation; The anxiety level of patientswith PCI operation in hospital can reduced on the day of and after the surgery, but itsintervention before PCI, still need clinical studies to be verified.3. On the basis of the current researches, optimizing the psychological nursing plan,constructing the psychological intervention mode on patients with anxiety during PCIperioperative period which adapt to Chinese Medical service system,in order to guideclinical practice. |
