ObjectiveThe study was to know about the incidence of cognitive dysfunction inpatients with symptomatic epilepsy, analysis of the structurecharacteristics of the cognition in patients with symptomatic epilepsy andits related factors, which provided significant reference basis for thecognitive status and quality of life in patients with epilepsy.MethodsCollected45cases of clinically diagnosed patients with symptomaticepilepsy from epilepsy center and neurosurgery of the first hospital ofJilin university, recorded the ages, primary diseases, frequencies,durations, types, medication, electroencephalogram and imaging datas ofthe patients, at the same time, collected48cases of healthy people fromthe first hospital of Jilin University, used《Wechsler Intelligence Scale ForAdult–Chinese Revised》to determine their intelligences, used statisticalknowledge to analyzes the related factors of cognition and symptomaticepilepsy.Results The patients of the research are chosen from epilepsy center andneurosurgery of the first hospital of Jilin university,a total of45cases,the biggest onset age was63years old, the smallest onset age is13yearsold, the average age was30.91±14.43years old. The longest duration was30years, the shortest duration was less than1year, the average durationwas8.9±8.15years.Of8cases of patients was simple partial seizures alone, accounted for18%, a total of23patients was complex partial seizures, accounted for51%, generalized seizures was a total of14cases, accounted for31%.9cases of cerebrovascular diseases, accounted for20%,13cases ofintracranial placeholder, accounted for29%,14cases of the braininjured, accounted for31%,5cases of intracranial infection,accounted for11%,4cases of blood sugar abnormal,accounted for9%.Frontal lobe discharge, a total of23cases (51%), temporal lobedischarge, a total of22cases(49%). Seizure frequency less than1timesa month was26cases, accounted for58%, seizure frequency is more than1times a month was19cases, accounted for42%.Compare with normal group, cases group operation IQ was88.51±14.3,verbal IQ was88.56±9.05, the total IQ was87.51±10.30,normal groupoperation IQ was102.17±7.92, verbal IQ was97.52±5.04, the total IQwas99.37±5.73P<0.05,there was statistically significant, whichindicated that case group has lower intellgence. Independent sample t test showed that11points test, arithmetic, digital span, words, numbers,symbols, mapping, arrange block diagram, pictures and graphics togethercompared with normal group, P<0.05, there was statistically significance.Knowledge, understanding, similarity has no statistical significance, alsowas lower than those of normal group.8cases of simple partial seizures,the verbal IQ was89.5±7.45, operation IQ was86.88±10.79, the total IQwas87.38±8.38.23cases of complex partial seizures, the verbal IQ was89.30±11.23, operation IQ was90.17±15.62, the total IQ was89.61±10.33.14cases of generalized seizures, the verbal IQ was85.0±8.54, operation IQ was90.17±15.62,the total IQ was84.14±10.95,P<0.05, there was statistically significance. Generalized seizures havemore influence on IQ than the others. Cognitive function in patients withdifferent primary diseases, intracranial infection, intracranial placeholdertotal IQ classification is lower than usual, has a greater influence on IQ.Relationship of the different parts of the epilepsy and cognitive functions:frontal lobe epilepsy operation IQ was statistically significance, verbal IQand total IQ had no statistically significance, suggested that frontal lobeepilepsy has a greater influence on operation IQ.The single factor correlation analysis indicated that the course of thedisease, onset age had relationship with total IQ level, Course wasassociated with verbal IQ and operation IQ, onset age, differentpathological had relationship with operating IQ, there was no obvious correlation with the verbal IQ. Education degree has correlation withverbal IQ, forms has correlation with total IQ, Age, frequency andprimary diseases had no obvious correlation with total IQ, operation IQ,verbal IQ. Linear regression analysis showed that epilepsy cognitivefunction had a significant relationship with the course of the disease, ageand education.Conclusion1The cognition dysfunction of symptomatic epilepsy damage isextensive and comprehensive.2Generalized seizures have a great influence on verbal IQ, operationIQ and total IQ.3Cerebrovascular diseases, intracranial infection have a greatereffection on verbal IQ, operation IQ than intracranial placeholder, bloodsugar abnormal and brain injured.4The frontal lobe epilepsy have significantly influence on operationIQ than temperal lobe epilepsy.5The higher seizure frequency,the lower verbal IQ, operation IQ andtotal IQ.6The longer course,the lower IQ,the greater,the lower orerationIQ,the greater education degree,the higher verbal IQ.
